Article Abstract

Immunoglobulin G4 (IgG4)-related disease is a fibroinflammatory condition involving diverse organs. We report a case of IgG4-related pancreatitis and retroperitoneal fibrosis with serial 68Ga-FAPI PET/CT scans after treatment. A 64-year-old man presented with left flank and epigastric pain. Laboratory, abdominal CT, and 68Ga-FAPI PET/CT findings were suggestive of IgG4-related pancreatitis and retroperitoneal fibrosis. Histology of the pancreas confirmed IgG4-related pancreatitis. The follow-up PET/CT scans after treatment with steroid therapy showed regression of 68Ga-FAPI uptake in the pancreas and periureteral soft tissue. The changes on 68Ga-FAPI PET/CT scans were much more prominent compared with the CT scans.

A 70-year-old man presented with progressive dysuria. 18F-FDG PET/CT showed no abnormal metabolic activity. 68Ga-FAPI PET/CT revealed intense 68Ga-FAPI in the lower left ureter and the left rib, mimicking malignancy with bone metastasis.

Primary splenic angiosarcoma is a rare, aggressive tumor, characterized by rapid metastasis and poor prognosis. Herein, we report a case of 18F-FDG and 68Ga-FAPI-04 PET/CT imaging in a 57-year-old man with primary splenic angiosarcoma. 68Ga-FAPI-04 PET/CT presented much higher uptake in the lesions compared with 18F-FDG PET/CT, showing better diagnostic performance.

Xanthogranulomatous cholecystitis (XGC) is an uncommon inflammatory disease of the gallbladder. We report a case of XGC with elevated activity of 18F-FDG and 68Ga-FAPI (fibroblast activating protein inhibitor), which mimics malignancy. Our case suggests that XGS should be considered in the differential diagnosis of cancer-like gallbladder lesions with increased 68Ga-FAPI uptake.

A 59-year-old man with metastatic melanoma and brain metastases underwent brain radiotherapy. Following treatment, he developed neurological symptoms, prompting further evaluation. To assess the treatment response and possibility of Lutetium-177-FAPI therapy, both 18F-FDG PET/CT and 68Ga-FAPI-46 PET/CT scans were performed.

Background: The fibroblast activation protein α (FAP)-directed radiotracer [Ga]Ga-FAPI-46 for PET-CT has shown promising diagnostic accuracy in cancer staging in retrospective studies. We aim to investigate the positive predictive value (PPV) of [Ga]Ga-FAPI-46 PET for detecting FAP-expressing tumours and the potential association between PET radiotracer uptake intensity and immunohistochemical FAP expression.

Methods: This single-centre, single-arm, interventional, phase 2 trial was conducted at the University Hospital Essen, Essen, Germany.
