Medications contribute to about a quarter of acute kidney injury (AKI) cases among patients in hospitals. The impact of AKI is substantial on both families and society, and it has become a worldwide public health concern. Recently, a new framework for drug-induced acute kidney injury (DI-AKI) classification has been proposed. According to this new framework, drugs are divided into four categories. Thus, we explain the mechanism thoroughly and give examples of drugs or drug categories linked to the classes in the new framework. Furthermore, a patient's condition may dynamically shift between categories. At the same time, we also took into account some susceptibility factors. These susceptibility factors may drive inter-class variation. The new classification system may shed new light on the mechanism of DI-AKI for clinicians and researchers.

Background: In hyponatremic patients, concurrent dialysate flow during hemodialysis may be an ideal option to mitigate complications such as osmotic demyelination syndrome (ODS).
Methods: Present randomized controlled trial enrolled dialysis-requiring chronic kidney disease (CKD) and acute kidney injury (AKI) patients with serum sodium levels < 125 mEq/L during January 2020 over 16 months. Hemodynamically unstable patients, as well as those with a history of seizures and neurological conditions, were excluded.

Diabetic kidney disease (DKD), a prominent microvascular complication of diabetes mellitus and the leading cause of end-stage renal disease (ESRD), was addressed through a novel nanotherapeutic approach. This study engineered folic acid-conjugated poly(lactic-co-glycolic acid) nanoparticles (FA-PLGA NPs) for the folate receptor (FR)-targeted delivery of Toll-like receptor 4 small interfering RNA (TLR4 siRNA) to treat diabetic nephropathy (DN). In a streptozotocin-induced DN murine model, administration of FA-PLGA NPs/TLR4 siRNA significantly mitigated renal injury compared to untreated DN controls.

Renal Resistive Index (RRI) and Renal Pulsatility Index (RPI) are currently used in the diagnosis of ureteral obstruction, early diagnosis and follow-up of acute kidney injury, assessment of chronic kidney disease, and evaluation of transplanted kidneys. However, their inter-observer and inter-scanner variability has not been investigated in dogs, limiting the accuracy and clinical applicability of these indices. The objectives of this cross-sectional observational prospective study were to assess the inter-observer and inter-scanner variability of RRI and RPI and to determine whether operator experience influences measurement accuracy.
