Importance: The association of cardiopulmonary point-of-care ultrasonography (POCUS) with length of stay (LOS) and hospitalization costs for patients admitted to internal medicine wards remains uncertain.
Objective: To evaluate a collaborative implementation model involving hospitalists, sonographers, and a remote cardiologist for integrating cardiopulmonary POCUS into the assessment of adult patients (≥18 years) hospitalized with undifferentiated dyspnea, and to assess its association with LOS and hospitalization costs.
Design, Setting, And Participants: This quality improvement study employed a type 1 effectiveness-implementation hybrid design using a 6-month stepped-wedge cluster randomized approach, conducted at a tertiary care hospital in the US between December 7, 2023, and July 2, 2024, to compare the standard-of-care (control) with the intervention group.

Methods: Data came from n=713 adult dual cigarette/ENDS users in the Population Assessment of Tobacco and Health Wave 6 public use data file. We used latent class analysis to identify qualitatively different subgroups within the sample and applied logistic regression to assess correlates of latent class membership.

Objective: Cannabis legalization may impact both cannabis and tobacco use, given the high prevalence of co-use (including blunt use) among young adults (YAs) in the United States. The objective of this descriptive ecological study was to examine trends in YA cannabis and tobacco use from 2002-2018 in states that passed adult and medical use (AMU) or medical use only (MUO) cannabis laws during that time ( = 16).
Method: Using data from the National Survey on Drug Use and Health, we conducted a segmented regression analysis to calculate absolute percent change in past 30-day cannabis, blunt, cigarette, and cigar use between time points.

Methods: We conducted a prospective, matched case-control study in which we measured pre-diagnostic metabolomic profiles among predominantly premenopausal women in the Nurses' Health Study II (n = 2010). Lipids, carbohydrates, and organic acid-related metabolites (n = 218) were profiled via liquid chromatography-tandem mass spectrometry.

Despite genome-wide association studies (GWAS) of late-onset Alzheimer's disease (LOAD) having identified many genetic risk loci, the underlying disease mechanisms remain largely unclear. Determining causal disease variants and their LOAD-relevant cellular phenotypes has been a challenge. Here, using our approach for identifying functional GWAS risk variants showing allele-specific open chromatin, we systematically identified putative causal LOAD-risk variants in human induced pluripotent stem (iPS)-cell-derived neurons, astrocytes and microglia, and linked a PICALM LOAD-risk allele to a microglial-specific role of PICALM in lipid droplet (LD) accumulation.

Introduction: Extremity vascular injuries (EVI) are rare, life-threatening injuries that may require ultra-massive transfusion (UMT), defined as transfusion of ≥20 units of red cell products in 24 hours. This study aimed to evaluate outcomes of patients with EVI who required UMT and to determine which variables are associated with survival.

Widespread yellow fever virus (YFV) immunity in Sub-Saharan Africa may mitigate orthoflavivirus outbreaks. Here, we investigate whether pre-existing YFV-17D immunity confers cross-protection against dengue virus serotype 2 (DENV-2) infection in a murine model. IFNAR1 mice immunized with YFV-17D exhibited significantly reduced DENV-2 viremia, weight loss, and disease severity, with improved survival compared to naïve controls.

Objective: Primary care-based literacy promotion enhances caregiver-child shared reading and child language outcomes, yet variation in implementation may dilute its impact. This study examines expert perspectives on intended outcomes of literacy promotion, as well as its core components, those necessary to achieve intended outcomes, and components that are recommended but adaptable to context.
Methods: We purposively sampled healthcare and policy experts in primary care-based literacy promotion from the U.
Objective: Deep brain stimulation (DBS) of the subcallosal cingulate (SCC) has been investigated clinically for treatment-resistant depression (TRD). Although the initial BROADEN study, a randomized controlled trial, was halted after an interim futility analysis, observation of long-term follow-up (LTFU) data from this and other cohorts demonstrated sustained improvement in depressive symptoms, prompting further investigation of DBS as a therapeutic option.
Methods: Data from 5 studies, including BROADEN, were used to evaluate the long-term safety and effectiveness of SCC DBS for TRD.

Introduction: Consistently delivering high-quality feedback is critical, yet challenging, in surgical education. We hypothesized that feedback quality, as assessed by artificial intelligence (AI, Teach1) based-tools, would correlate with improvements in technical skill acquisition during simulated paracentesis training.
Methods: Seventy-five fourth-year medical students meeting inclusion criteria (baseline Objective Structured Assessment of Technical Skills scores ≤20) performed paracentesis on simulators, received instructor feedback via a digital platform, and completed a second attempt.

Background And Aims: The TRISCEND II trial demonstrated superior clinical benefits for patients with ≥severe tricuspid regurgitation (TR) treated with the EVOQUE transcatheter tricuspid valve replacement (TTVR) system plus medical therapy versus medical therapy alone. This work reports 1-year and 18-month outcomes in patients stratified by baseline TR severity.
Methods: The multicentre, prospective TRISCEND II trial enrolled 400 patients with symptomatic, ≥severe TR and randomised 2:1 to TTVR (n=267) or control (n=133).
