A 65-year-old female patient with Sjögren syndrome (SJS) and tubulointerstitial nephritis (TIN) confirmed by kidney biopsy results 7 years previously presented to our department with progressive kidney function deterioration. Laboratory findings revealed increased serum creatinine level accompanied with deterioration of tubular function. Although she already had signs of proximal tubular dysfunction due to TIN from 7 years before, deterioration of the proximal tubule related parameters was particularly prominent. Owing to the substantial increase in the IgG level, monoclonal gammopathy was considered. Subsequent immunoelectrophoresis of the serum and urine revealed the presence of monoclonal IgG-κ. A repeat kidney biopsy revealed crystalline inclusions in the proximal tubules that tested positive for κ light chain. Along with findings from the bone marrow biopsy, this led to the diagnosis of light chain proximal tubulopathy (LCPT) secondary to multiple myeloma. Chemotherapy was initiated promptly which resulted in stabilization of renal function and improvement of tubular function. Vigilant monitoring of the tubular function is essential for patients with SJS. Deterioration of proximal tubule parameters, especially when accompanied by increased IgG levels, should prompt the investigation of monoclonal gammopathy and the consideration of kidney biopsy.

Dent disease is a rare X-linked recessive inherited renal tubular disorder characterized by low molecular weight proteinuria (LMWP), hypercalciuria, nephrocalcinosis, and other clinical features, and can lead to progressive renal failure. It is primarily caused by mutations in the gene. This article reports the case of a 10-year-old male patient of Chinese descent who was incidentally found to have asymptomatic proteinuria during a routine health examination.

Background: Calcineurin inhibitor (CNI) toxicity is a significant cause of graft dysfunction in kidney transplant recipients, yet distinguishing it from acute rejection (AR) and acute tubular necrosis (ATN) remains challenging. This study investigated the use of urinary mRNA biomarkers as a noninvasive tool for identifying CNI toxicity.
Methods: We retrospectively enrolled 110 kidney transplant recipients and classified them into four groups based on pathological findings: stable graft function (n=35), CNI toxicity (n=25), AR (n=30), and ATN (n=20).

The increasing presence of nanoplastics (NPs) and synthetic antioxidants like 3-tert -Butyl-4-hydroxyanisole (3-BHA) in the environment has attracted widespread attention about their combined toxicological effects on human health, particularly on renal function. This study explored to the combined impacts of NPs and 3-BHA at environmentally relevant concentrations on sub-chronic kidney injury in mice. Firstly, our results confirmed that the accumulation of 80 nm NPs in renal tissues, leading to structural abnormalities such as reduced mitochondrial cristae and increased empty bubbles in mice by transmission electron microscope (TEM) analysis.
