Pathogenic variants in the gene lead to a systemic disease with karyomegalic interstitial nephritis (KIN) at the forefront clinically. The phenotypic-genotypic features of a mutation-related disease involving five members of a Hungarian Caucasian family are presented. Each had adult-onset chronic kidney disease of unknown cause treated with renal replacement therapy and elevated liver enzymes. Short stature, emaciation, latte-colored skin, freckles, and a hawk-like nose in four patients, a limited intellect in two patients, and chronic restrictive lung disease in one patient completed the phenotype. Severe infections occurred in four patients. All five patients had ceased. Four patients underwent autopsy. KIN and extrarenal karyomegaly were observed histologically; the livers showed no specific abnormality. The genotyping using formalin-fixed tissue samples detected a hitherto undescribed homozygous mutation (c.1673_1674insT/p.Met558lfs*4; exon 5) in three of these patients and a heterozygous mutation in one patient. The reason for the heterozygosity is discussed. In addition, 56 family members consented to the screening for mutation from which 17 individuals proved to be heterozygous carriers; a blood chemistry evaluation of their kidney and liver function did not find any abnormality. The clinical presentation of FAN1-related disease was multifaceted, and not yet described manifestations were observed besides kidney and liver disease. Mutation in this gene should be suspected in adults with small kidneys of unknown cause, elevated liver enzymes, and recurrent infections, even without a family history.

Background: Cerebrotendinous xanthomatosis is a rare autosomal recessive lipid storage disorder involving bile acid biosynthesis. Reduced mitochondrial cytochrome P450 enzyme activity leads to abnormal lipid accumulation in various tissues, especially tendons, lenses, and the central and peripheral nervous systems. This condition manifests with systemic symptoms such as neurological disorders, atherosclerosis, tendon xanthomas, and cataracts.

FLCN-associated eosinophilic renal tumors mainly refer to hybrid oncocytic/chromophobe tumors (HOCT) and other oncocytic tumors related to Birt-Hogg-Dubé (BHD) syndrome, which can sometimes occur sporadically. Accurate diagnosis of FLCN-associated tumors is challenging due to their morphologic heterogeneity and the lack of reliable biomarkers. We evaluated the clinicopathologic and IHC profiles of 18 eosinophilic renal tumors with targeted DNA sequencing-confirmed FLCN mutations, including 10 typical HOCT and 8 unclassified tumors.

Background: To explore the clinical phenotypic characteristics and genetic analysis of children with SCN2A gene mutation-related epilepsy.
Methods: A retrospective study of children with SCN2A gene mutation epilepsy admitted to the Department of Neurology of Hebei Provincial Children's Hospital from January 2020 to May 2023 was conducted to analyze their clinical characteristics, treatment response, and prognosis, and to explore the correlation between mutation type and efficacy.
Results: The age of onset of disease in the 12 children with SCN2A was distributed from 22 h after birth to 10 years and 11 months of age, the neonatal missense mutation was the most common, followed by nonsense mutation, the type of seizure was predominantly generalized tonic-clonic seizures(Generalized tonic-clonic seizures, GTCS), paraventricular white matter echo enhancement was the most common of the magnetic resonance image(Magnetic resonance image, MRI), and electroencephalogram(Electroencephalogram, EEG) were predominantly spiking and spiking-slow wave issuance in all phases of wakefulness and sleep, and there was a generalized developmental disorder in 11 cases; of the 12 cases of the children, the diagnosis of epilepsy was diagnosed in 3 cases, and levetiracetam(Levetiracetam, LEV) treatment was ineffective in all of them; epilepsy syndrome was diagnosed in 9 cases, of which 4 cases of Dravet syndrome(Dravet syndrome, DS) had the highest number of cases, and LEV and valproate(Valproate, VPA) were effective in 50% of the children, and multiple medications were ineffective in the remaining 2 cases; 2 children with west syndrome(West syndrome, WS) were ineffective in the administration of topiramate(Topiramate, TPM), clonazepam(Clonazepam, CZP), and pro-adrenocorticotropic hormone, All have global developmental delays; 2 cases of ohtahara syndrome(Ohtahara syndrome, OS), 1 case of phenobarbital(Phenobarbital, PB) treatment was effective, 1 case was ineffective in multiple drug treatment; 1 case of self-limited epilepsy with centrotemporal spikes(self-limited epilepsy with centrotemporal spikes, SeLECTS) with normal development of central temporal spikes, LEV could control seizures.
