Despite the success of mRNA COVID-19 vaccines, extending this modality to more diseases necessitates substantial enhancements. We present GEMORNA, a generative RNA model that utilizes Transformer architectures tailored for mRNA coding sequences (CDSs) and untranslated regions (UTRs), to design novel mRNAs with enhanced expression and stability. GEMORNA-designed full-length mRNAs exhibited up to a 41-fold increase in firefly luciferase expression compared to an optimized benchmark in vitro.

Background: Irregular eating patterns, such as skipping breakfast and late dinner consumption, have been linked to adverse metabolic profiles. However, the impact of these behaviors or their combined effect on metabolic dysfunction-associated steatotic liver disease (MASLD) remains unclear.

Methods: In this multicenter cross-sectional study, we analyzed data from 1965 adults recruited from diabetes, obesity, and metabolic disease clinics in six municipalities/provinces in China.

Study Objectives: Excessive daytime sleepiness (EDS), influenced by environmental and social-behavioral factors, is reported by a subset of patients with sleep apnea - a group that may be at elevated cardiovascular risk. However, it is unclear whether sleep apnea with and without EDS have distinct genetic underpinnings. In this study, we perform gene-by-EDS interaction analyses for apnea hypopnea index (AHI), a diagnostic marker of sleep apnea severity, to understand EDS's influence on its underlying genetic risk.

The gut microbiota, a complex microbial ecosystem within the gastrointestinal (GI) tract, plays a pivotal role in maintaining GI homeostasis. Dysbiosis of this community is increasingly implicated in the pathogenesis of diverse GI disorders. Bacterial extracellular vesicles (bEVs) secreted from gut microbes have emerged as an innovative therapeutic nanoplatform for GI diseases.

Background: Human T-cell lymphotropic virus-1(HTLV-1) infection may lead to adult T-cell leukemia, HTLV-associated myelopathy/tropical spastic paraplegia, and various neurological diseases. Therefore, developing a rapid and accurate detecting method is crucial for preventing and controlling HTLV-1 infection. This study aims to develop and validate a detection assay for HTLV-1 proviral DNA by combining multienzyme isothermal rapid amplification(MIRA) and lateral flow dipstick(LFD) techniques.

Background: Autoimmune hepatitis (AIH) is a chronic liver disease characterized by immune-mediated liver inflammation. Despite its global prevalence, the pathogenesis of AIH remains poorly understood, and there is a lack of specific biomarkers and targeted treatments. This study aimed to investigate the role of hsa_circ_0109623, hsa-miR-146b-3p, and Sortilin 1 (SORT1) in AIH and their potential as therapeutic targets.

Background: This study aimed to observe corneal and retinal thicknesses at 5 years after femtosecond laser-assisted in situ keratomileusis (FS-LASIK) and small incision lenticule extraction (SMILE) for myopia, investigate the effect of epithelial remodeling on refractive status and visual quality, and compare retinal thicknesses among fundus tessellation grades.

Methods: Patients who received FS-LASIK or SMILE 5 years before were enrolled in this cross-sectional study. After 1:1 propensity score matching, each surgical group obtained 177 patients (177 eyes).

Background: With the fast pace of modern life, people have less time for meals, but few studies have examined the association between the habit of fast eating and metabolic diseases.

Objective: Combining the results of the current study and the prior ones, we aimed to investigate the possible relationship between fast eating and the risk of metabolic dysfunction-associated steatotic liver disease (MASLD).

Methods: This is a sub-analysis of a multicenter cross-sectional study of 1965 participants investigated the association between fast eating and MASLD in Chinese.

Viral infections are major causes of mortality in solid-organ and hematopoietic stem cell transplant recipients. Epstein-Barr virus (EBV) and Parvovirus B19 (B19V) are among the common viral infections after transplantation and were recommended for increased screening in relevant guidelines. Therefore, the development of rapid, specific, and cost-effective diagnostic methods for EBV and B19V is of paramount importance.

Purpose: The corneal cap thickness is a vital parameter designed in small incision lenticule extraction (SMILE). The purpose was to investigate the changes in corneal subbasal nerve plexus (SNP) and stromal cells with different cap thicknesses and evaluate the optimized design for the surgery.

Methods: In this prospective, comparative, non-randomized study, a total of 108 eyes of 54 patients who underwent SMILE were allocated into three groups with different corneal cap thicknesses (110 μm, 120 μm or 130 μm group).

Article Synopsis
  • A new multiplex real-time PCR technique was created to detect cytomegalovirus (CMV), Epstein-Barr virus (EBV), and herpes simplex virus (HSV) together, while also checking sample quality.
  • Testing on 500 blood donors and 320 transfusion recipients showed the assay had a high sensitivity of 100 copies/μL and was 100% consistent with existing commercial tests.
  • Out of 820 blood samples, the assay found 34 positive for CMV, 15 for EBV, and 6 for HSV, suggesting that virus testing should be standard for blood transfusions, especially in high-risk groups.

CD4 T cells play a critical role in the pathogenesis of viral diseases, which are activated by the internal metabolic pathways encountering with viral antigens. Glutaminolysis converts glutamine into tricarboxylic acid (TCA) circulating metabolites by α-ketoglutaric acid, which is essential for the proliferation and differentiation of CD4 T cells and plays a central role in providing the energy and structural components needed for viral replication after the virus hijacks the host cell. Changes in glutaminolysis in CD4 T cells are accompanied by changes in the viral status of the host cell due to competition for glutamine between immune cells and host cells.

Objectives: To assess the need for screening of transfusion-transmitted infections (TTIs) in blood products, we assessed TTI seroprevalence in blood donors and hospitalized patients.

Methods: We collected 2760 serum samples from three regions of Hangzhou, Ningbo and Huzhou from April 2021 to March 2022, and they tested by enzyme-linked immunosorbent assay (ELISA) for Hepatitis B surface antigen (HBsAg), Hepatitis C (HCV), Treponema pallidum (TP), Cytomegalovirus (CMV), Epstein-Barr virus (EBV), Hepatitis E virus (HEV) and Human T-cell lymphotropic virus type 1/2 (HTLV-1/2) antibody levels.

Results: Screening test results showed that the positive rates for HBsAg, anti-HCV and anti-TP were 3.

  • The study aimed to examine how high blood sugar levels affect kidney function decline (measured by eGFR) in older adults.
  • A comparison of diabetic and non-diabetic individuals revealed that diabetes significantly worsens kidney function decline over four years, with diabetic patients experiencing a 9.29% drop in eGFR versus 5.32% in non-diabetics.
  • The findings highlight a growing prevalence of diabetes among the elderly, rising from 12.67% to 16.68% between 2017 and 2021, and the importance of monitoring kidney health in this group.

Article Synopsis
  • The study investigates the role of PD-1 expression in tumor-associated macrophages (TAMs) and its impact on osteosarcoma (OS) progression, particularly in lung metastases.
  • Using single-cell RNA sequencing, researchers identified various cell clusters within human OS metastatic lesions, revealing an increase in exhausted T cells and regulatory T cells while noting that T cells in metastases proliferate less than those in primary tumors.
  • The findings indicate that the immunosuppressive M2 phenotype of TAMs in OS metastasis hinders their ability to eliminate cancer cells, highlighting the potential of PD-1 inhibitors as a therapeutic approach for OS.
