Reports on surgical treatment for postherpetic trigeminal neuralgia after COVID vaccination have not been found in the literature. Here, we described a case of postherpetic trigeminal neuralgia after COVID vaccination that was treated with percutaneous retrogasserian glycerol rhizotomy (PRGR) resulting in complete pain relief. We reported a case involving a 70-year-old female who had herpes zoster infection in the ophthalmic branch (V1) dermatome following COVID vaccination and presented with paroxysmal electric shock-like sensation without a trigger.

Endovascular therapy (EVT) has transformed the acute management of large vessel occlusion strokes, with strong evidence from landmark trials validating its effectiveness. However, numerous clinical and procedural controversies remain unresolved ranging from patient selection and treatment windows to management of intracranial atherosclerotic disease and reperfusion-related injury. This editorial critically examines these debates, integrating trial data, mechanistic insights, and expert commentary.

Classic clinical models based on factors such as size and location have demonstrated limited predictive accuracy, with small aneurysms being capable of rupture and larger ones remaining stable. Recent advances in Artificial Intelligence (AI) now allow the development of more sophisticated models that integrate both geometric and hemodynamic variables, including wall shear stress (WSS) and blood flow dynamics.

Introduction: The clivus is an uncommon site for fungal infections and is typically associated with tumors or metastases. Invasive fungal sinusitis extending to the skull base is exceptionally rare and often mimics clival malignancies such as chordomas or metastases. This overlap in clinical and radiological features can lead to diagnostic delays.

Brain magnetic resonance imaging (MRI) with contrast showed a left frontal space-occupying lesion, suggestive of a high-grade malignancy. Awake craniotomy with complete excision of the lesion was performed under immunofluorescence guidance.

Background: Chronic subdural hematoma (CSDH) is a prevalent neurosurgical condition characterized by blood accumulation within the border cell layer. Despite various treatment options-medical, endovascular, and surgical-recurrence rates remain high. Our pilot study investigates the safety and efficacy of endoscopic membranectomy (EM) in reducing recurrence in nonhomogenous CSDH.

Context: Vertex epidural hematoma (VEDH) is uncommon. A high index of suspicion is required to suspect and diagnose this condition, and the surgical management is a challenge to neurosurgeons. There are only isolated case reports or small series of VEDH in the literature.

Intramedullary spinal cavernomas are relatively rare with only 24 cases reported till date to the best of our knowledge. A 15 -year-old boy presented to the clinic with acute onset motor weakness in all four limbs.
