Understanding speech in noise is a common challenge for older adults, often requiring increased listening effort that can deplete cognitive resources and impair higher-order functions. Hearing aids are the gold standard intervention for hearing loss, but cost and accessibility barriers have driven interest in alternatives such as Personal Sound Amplification Products (PSAPs). While PSAPs are not medical devices, they may help reduce listening effort in certain contexts, though supporting evidence remains limited.

Treatment of type 2 diabetes in pregnancy: Do we need to use metformin?

Best Pract Res Clin Endocrinol Metab

August 2025

Division of Endocrinology & Metabolism, Department of Medicine, University of Toronto, Toronto, Canada; Mount Sinai Hospital, Sinai Health System, Toronto, Canada; Lunenfeld-Tanenbaum Research Institute, Toronto, Canada. Electronic address:

Maternal type 2 diabetes (T2D) is associated with an increased risk of several pregnancy complications and its prevalence is rising globally. Metformin, an oral medication that is first-line therapy for T2D outside of pregnancy, has the potential to improve glycemic control and reduce insulin requirements, without contributing to maternal hypoglycemia or weight gain. However, it crosses the placenta, and up until recently, very few clinical trials existed that studied its role in T2D pregnancies.

Background: Composite outcomes in cardiovascular trials often group events of unequal clinical importance, and conventional analyses may obscure treatment trade-offs. Generalised pairwise comparisons (GPC), expressed as a win ratio (WR), allow for hierarchical ranking of events and incorporation of recurrent outcomes, providing a potentially more intuitive assessment of benefit-risk.

Methods: In a prespecified exploratory analysis of the 2×2 factorial, randomised CLEAR (Colchicine and Spironolactone in Patients with Myocardial Infarction) trial (7062 patients within 72 hours of acute myocardial infarction (MI) and percutaneous coronary intervention), we applied both time-to-first and recurrent-event GPC to reassess low-dose colchicine (0.

Patient-Reported Symptoms and Direct Health Care Costs in Head and Neck Cancer.

JAMA Otolaryngol Head Neck Surg

September 2025

Institute of Health Policy, Management, and Evaluation, Dalla Lana School of Public Health, University of Toronto, Toronto, Ontario, Canada.

Importance: Head and neck cancer (HNC) and its associated treatments are associated with substantial functional, psychological, and financial consequences. Patient-reported outcome measures (PROMs) play a crucial role in capturing the full impact of disease. Understanding how PROMs are associated with health care costs is critical for cancer care planning; however, the association of health care expenditure and PROMs is yet to be clarified.

Background: Radiation therapy is a common treatment for head and neck (H&N) cancers. Radiomic features, which are determined from biomedical imaging, can be effective biomarkers used to assess tumor heterogeneity and have been used to predict response to treatment. However, most studies employ only a single biomedical imaging modality to determine radiomic features.

Emerging pharmacotherapies for brain metastases: a spotlight on angiogenesis inhibitors.

Expert Opin Pharmacother

September 2025

Division of Neurosurgery, Department of Surgery, Temerty Faculty of Medicine, University of Toronto, Toronto, ON, Canada.

Introduction: Brain metastases represent a major cause of morbidity and mortality in cancer patients and are challenging to manage due to the protective blood-brain barrier and the aggressive nature of intracranial disease. Angiogenesis, particularly mediated by vascular endothelial growth factor (VEGF) and integrin pathways, plays a critical role in the growth and progression of brain metastases. Inhibiting angiogenesis has emerged as a therapeutic strategy to control tumor progression, reduce edema, and improve clinical outcomes.
