Digital twins (DTs) are emerging tools for simulating and optimizing therapeutic protocols in personalized nuclear medicine. In this paper, we present a modular pipeline for constructing patient-specific DTs aimed at assessing and improving dosimetry protocols in PRRT such as therapy. The pipeline integrates three components: (i) an anatomical DT, generated by registering patient CT scans with an anthropomorphic model; (ii) a functional DT, based on a physiologically-based pharmacokinetic (PBPK) model created in SimBiology; and (iii) a virtual clinical trial module using GATE to simulate particle transport, image simulation, and absorbed dose distribution.

Evaluation of tumor infiltrating lymphocytes as recommended by current guidelines is largely based on stromal regions within the tumor. In the context of epithelial malignancies, the epithelial region and the epithelial-stromal interface are not assessed, because of technical difficulties in manually discerning lymphocytes when admixed with epithelial tumor cells. The inability to quantify immune cells in epithelial-associated areas may negatively impact evaluation of patient response to immune checkpoint therapies.

Background: In contrast-enhanced digital mammography (CEDM) and contrast-enhanced digital breast tomosynthesis (CEDBT), low-energy (LE) and high-energy (HE) images are acquired after injection of iodine contrast agent. Weighted subtraction is then applied to generate dual-energy (DE) images, where normal breast tissues are suppressed, leaving iodinated objects enhanced. Currently, clinical systems employ a dual-shot (DS) method, where LE and HE images are acquired with two separate exposures.

Functional magnetic resonance imaging (fMRI) is a pivotal tool for mapping neuronal activity in the brain. Traditionally, the observed hemodynamic changes are assumed to reflect the activity of the most common neuronal type: excitatory neurons. In contrast, recent experiments, using optogenetic techniques, suggest that the fMRI-signal could reflect the activity of inhibitory interneurons.
