Background: The clinical impact of rare epidermal growth factor receptor (EGFR) exon 19 insertion-deletion (19delins) variants on tyrosine kinase inhibitor (TKI) efficacy remains poorly characterized. We updated 5-year outcomes to evaluate long-term survival and optimal treatment strategies in advanced lung adenocarcinoma (LUAD) patients harboring these mutations.
Materials And Methods: In this multicenter prospective study, 36 treatment-naive advanced LUAD patients with EGFR 19delins mutations received first-generation (n = 26) or third-generation TKIs (n = 10).

: Gefitinib is a first-generation epidermal growth factor receptor tyrosine kinase inhibitor (EGFR-TKI) targeting EGFR-mutated non-small cell lung cancer (NSCLC) and is a current first-line treatment for NSCLC. Compared to exon 19 deletions, the epidermal growth factor receptor (EGFR) exon 21 L858R (21L858R) mutation is associated with a poorer prognosis in non-small cell lung cancer (NSCLC), particularly in patients with brain metastases (BMs). However, there is a notable lack of prospective or retrospective clinical studies focusing on this specific population. This study aims to evaluate the efficacy of different first-line treatment strategies using EGFR-tyrosine kinase inhibitors (TKIs) in NSCLC patients harboring the 21L858R mutation and BMs while providing insights into concurrent mutations and resistance mechanisms.

Background: While perioperative immunotherapy and adjuvant targeted therapy have improved outcomes for advanced non-small cell lung cancer (NSCLC), evidence on preoperative targeted strategies remains limited. This study retrospectively evaluated the efficacy and safety of neoadjuvant epidermal growth factor receptor tyrosine kinase inhibitor (EGFR-TKI) therapy with or without chemotherapy in resectable EGFR-mutant NSCLC.
Methods: Consecutive patients with EGFR-mutant NSCLC undergoing preoperative EGFR-TKI monotherapy or EGFR-TKI plus platinum-based chemotherapy followed by surgical resection were identified from three Chinese thoracic surgery prospectively maintained databases (2010-2023) from Peking University Cancer Hospital, Sun Yat-sen University Cancer Center, and The Affiliated Hospital of Putian University.
