Given its crucial role in determining patient symptoms and outcomes in various cardiopulmonary diseases, the thorough and accurate assessment of right ventricular function is essential for both diagnosis and ongoing patient monitoring. In the era of precision medicine, a more detailed characterization of patients with cardiopulmonary diseases is needed, especially with the emergence of novel pharmacological and device-based therapies, such as transcatheter tricuspid valve intervention, gene therapy in patients with cardiomyopathy and anti-obesity interventions for patients with heart failure. Precise and reproducible quantification of right ventricular morphology and function are crucial for risk stratification, the selection of different therapies for the appropriate patients and the evaluation of treatment outcomes. As our understanding of right ventricular pathophysiology expands, the need for sensitive markers of functional deterioration, reliable prognostic indicators and more precise surrogates for clinical trials becomes increasingly important. In this Roadmap, we address current challenges in the standardization of image acquisition, analysis and interpretation across different modalities. We explore the factors limiting the clinical adoption of more advanced approaches and provide expert recommendations to overcome these barriers. Additionally, we outline potential next steps for incorporating parameters of right ventricular function as surrogate end points in multicentre clinical trials of new drugs or devices, and highlight new research opportunities, including the integration of artificial intelligence technologies. Finally, we issue a call for international collaboration on selected priority areas.

Materials And Methods: This retrospective cohort study used data from the IQVIA Disease Analyzer database, comprising anonymized records from over 3000 office-based physicians in Germany. We included 233 730 patients aged ≥40 years with at least one recorded BMI measurement between January 2005 and December 2023.

Objective: To evaluate the efficacy of baricitinib in combination therapy for managing refractory, rapidly progressive systemic sclerosis (SSc) with severe cardiac conduction defects and interstitial lung disease (ILD).
Methods: A 48-year-old male patient with SSc complicated by significant cardiac enlargement, third-degree atrioventricular block, heart failure, progressive ILD, and partial intestinal obstruction was included in the study. Prior treatments with mycophenolate mofetil (MMF), tacrolimus, and cyclophosphamide (CTX) had shown limited efficacy.

Objectives: Unstructured data, such as procedure notes, contain valuable medical information that is frequently underutilized due to the labor-intensive nature of data extraction. This study aims to develop a generative artificial intelligence (GenAI) pipeline using an open-source Large Language Model (LLM) with built-in guardrails and a retry mechanism to extract data from unstructured right heart catheterization (RHC) notes while minimizing errors, including hallucinations.
Materials And Methods: A total of 220 RHC notes were randomly selected for pipeline development and 200 for validation from the Pulmonary Vascular Disease Registry.

Common pulmonary vein atresia (CPVA) is a rare congenital heart disease characterized by the absence of functional connectivity between the pulmonary vein and any other heart cavity or systemic venous structure. A 13-h-old newborn (G3P3) was admitted to the department of pediatrics of a local maternity hospital and given tracheal intubation ventilator for assisted breathing due to systemic cyanosis, respiratory distress, and poor response 4 h after birth. He was transferred to Handan Maternal and Child Health Hospital 7 h after birth.

Background: Rheumatoid arthritis-associated interstitial lung disease (RA-ILD) portends a devastating prognosis for patients, with survival typically being < 5 to 8 years after diagnosis. Limited clinical trial data exist to guide treatment strategies, and the efficacy of current strategies-immunomodulation and antifibrotics-remains uncertain. Large randomized controlled trials are costly, but pragmatic trial designs could reduce expenses.
