Purpose: Many surgical techniques for isolated gastrocnemius recession have been described. The purpose of the present study is to compare their therapeutic efficacy and intrinsic stability on a fresh cadaveric model of gastrocnemius tightness.
Methods: The cadaveric model was established by distracting the knee with spacers composed of low-temperature thermoplastic material, and was identified by the Silfverskiöld test. Procedures of gastrocnemius recession described by Barouk, Baumann and Strayer were performed. The lengthening distance and improvement of ankle dorsiflexion were measured.
Results: The mean ankle angle of the cadaveric models was -15.4° with the knee fully extended, and 8.4° with the knee flexed. The increased angle achieved by the Strayer procedure was significantly greater than that achieved by the Barouk procedure and the Baumann procedure with one cut (P < 0.05), but similar to that of the Baumann procedure with two cuts (P > 0.05). Compared with the intramuscular lengthening of the gastrocnemius (Baumann and Barouk procedures), the stability of the lengthening after the Strayer procedure was the lowest.
Conclusion: The three techniques of isolated gastrocnemius recession have different efficacy and stability in cadaver trials, depending on their own anatomical characteristics. Our study supplies an anatomical guide for the selection of a proper procedure.

Background: Convergence insufficiency intermittent exotropia (CIX(T)) is a common type of strabismus in children, characterized by greater ocular deviation at near fixation compared to distance fixation. This study aimed to explore impact of slanted lateral rectus recession (S-LRc) compared to conventional lateral rectus recession (LR) on pediatric patients with CIX(T).
Methods: This retrospective study enrolled pediatric patients with CIX(T) at Shanxi Aier Eye Hospital between June 2022 and December 2024.

We present a rare case of a middle-aged male with his eyes fixed in hypotropia-esotropia for 12 years. He had been diagnosed with and operated on for posterior fossa pilocytic astrocytoma prior to symptom onset. Contrast-enhanced MRI of the brain and orbit revealed dilated ventricles, a shunt in situ and a distorted midbrain architecture.

BACKGROUND Strabismus, eye misalignment or squint, is constant or intermittent, and may be associated with refractive changes and amblyopia (lazy eye). These factors determine the approach to surgical correction. The present study aimed to retrospectively analyze type composition, refractive status, and surgical methods used with strabismus inpatients, to provide some reference for clinical practice.

Background: Muscle parameters are closely related to the efficacy of neoadjuvant therapy, and this study aims to analyze the predictive value of a novel muscle parameter index (skeletal muscle gauge, SMG) for the efficacy of neoadjuvant chemotherapy and immunotherapy(NCI) in locally advanced gastric cancer(LAGC).
Method: A retrospective analysis was conducted on the clinical pathological data and abdominal CT images of patients with LAGC who received NCI at the Second Affiliated Hospital of Nanchang University from September 2020 to December 2023. The body composition, including skeletal muscle index (SMI), skeletal muscle radiation attenuation (SMRA), and the new skeletal muscle parameter- SMG (obtained by multiplying SMI and SMRA), was calculated using the SliceOmatic software before and after NCI.

Recent anatomic and functional studies of the extraocular muscles have led to a new understanding that their proper function is dependent upon a complex collagenous orbital scaffolding. This structure, for which the term "pulley system" is a useful concise designation, maintains the positions of the eye muscles relative to the globe, to each other, and to the orbit. Disruption of this system results in muscle displacement and dysfunction.
