Article Abstract

We present a rare case of a middle-aged male with his eyes fixed in hypotropia-esotropia for 12 years. He had been diagnosed with and operated on for posterior fossa pilocytic astrocytoma prior to symptom onset. Contrast-enhanced MRI of the brain and orbit revealed dilated ventricles, a shunt in situ and a distorted midbrain architecture. The recti did not show any displacement, and axial length was not elongated. It was hypothesised that damage to vertical gaze centres, in conjunction with lateral rectus palsy due to protracted hydrocephalus, caused the clinical presentation. Surgical treatment involved bilateral medial rectus recession of 4.5 mm and inferior rectus recession of 7 mm to restore near orthotropia. To the best of our knowledge, this is a novel case of strabismus fixus due to coexisting upgaze palsy and sixth nerve palsy.

Purpose: The study aims to systematically review and analyze surgical treatments for Heavy Eye Syndrome (HES), focusing on outcomes and advantages of different procedures, to address the lack of consensus on its management and complications.

Methods: Following PRISMA guidelines (PROSPERO: CRD42024584537), PubMed, MEDLINE, Scopus, and CINAHL were searched up to October 2024 for studies addressing surgical management of HES. A total of 555 studies were identified, with 25 included in the systematic review, of which 19 were included in the meta-analysis.

Acquired strabismus in high myopia is typically fixed in the positions of adduction and depression, with restrictions in both abduction and elevation. As a treatment for myopic strabismus fixus, the Yokoyama procedure is effective. We report a case of strabismus fixus with a long axial length (34 mm), in which abduction limitation was improved by the Yokoyama procedure with medial rectus recession.
