Article Abstract

Purpose: Quadripolar left ventricular (LV) leads allow for several pacing configurations in candidates for cardiac resynchronization therapy (CRT). Whether different pacing configurations may affect LV dyssynchrony and systolic function is not completely known. We aimed to evaluate the acute effects of different pacing vectors on LV electromechanical parameters in patients implanted with a quadripolar LV lead.

Methods: In this two-centre study, within 1 month of implantation 21 CRT patients (65 ± 8 years, 76 % men, 38 % ischemic) receiving a quadripolar LV lead (Quartet 1458Q, St Jude Medical) underwent LV capture threshold assessment, intracardiac electrogram optimization, and two-dimensional echocardiography during four pacing configurations: D1-P4, P4-RV coil, D1-RV coil, and P4-M2. LV dyssynchrony and contractile function were expressed by septal-to-lateral delay and global longitudinal strain (GLS).

Results: LV capture threshold varied between the configurations (P < 0.001), showing higher values in the configurations P4-RV coil and P4-M2. Septal-to-lateral delay decreased in the configurations D1-P4 and D1-RV coil (P = 0.003 and P = 0.033 vs. spontaneous rhythm, respectively). GLS improved significantly vs. spontaneous rhythm only in the configuration D1-P4 (from -8.6 ± 3.5 to -11.0 ± 3.2 %, P = 0.001). Accordingly, an increase in stroke volume and a decrease in mitral regurgitation were observed in the configuration D1-P4 (P ≤ 0.001 vs. spontaneous rhythm).

Conclusions: In CRT patients receiving a quadripolar LV lead, significant variations in electromechanical parameters were observed by changing pacing vector. Individually targeting the optimal pacing site may enhance the acute haemodynamic response to CRT.

The integration of AI chatbots in nursing education, particularly in simulation-based learning, is advancing rapidly. However, there is a lack of structured evaluation models, especially to assess AI-generated simulations. This article introduces the AI-Integrated Method for Simulation (AIMS) evaluation framework, a dual-phase evaluation framework adapted from the FAITA model, designed to evaluate both prompt design and chatbot performance in the context of nursing education.

Cardiovascular diseases are the leading cause of death. Clinical data used to decide treatment are hard to integrate and interpret, making optimal treatment selection difficult. Personalised models can be used to integrate clinical data into a physics and physiology-constrained framework, but their clinical application faces limitations due to complex calibration and validation.

Background: Uninterrupted left bundle branch pacing (LBBP) lead implantation with the endpoint of transition to selected (S)-left bundle (LB) capture is feasible and safe during the procedure; however, the relatively long-term outcomes remain unknown.

Objective: The present study aimed to evaluate the short- to mid-term performance and safety of uninterrupted LBBP lead implantation.

Methods: Patients who underwent successful LBBP and achieved the implantation endpoint, guided by continuous paced intracardiac electrogram monitoring, were enrolled.

Background: Ventricular arrhythmias are a significant risk for patients who have suffered a myocardial infarction, with cardiac scar remodeling playing a critical role in arrhythmia development. Understanding the structural and electrical properties of the scar and its surrounding tissue is essential for assessing arrhythmia risk.

Objective: This study aimed to investigate the role of scar anatomy and border zone properties in predicting ventricular arrhythmias, using patient-specific three-dimensional models derived from medical imaging.

Proper nutrition is a fundamental aspect to maintaining overall health and well-being, influencing both physical and social aspects of human life; an unbalanced or inadequate diet can lead to various nutritional deficiencies and chronic health conditions. In today's fast-paced world, monitoring nutritional intake has become increasingly important, particularly for those with specific dietary needs. While smartphone-based applications using image recognition have simplified food tracking, they still rely heavily on user interaction and raise concerns about practicality and privacy.
