In this article, a case of decubitus ulcer infection caused by an extended-spectrum beta-lactamase-producing Proteus mirabilis strain, successfully treated with oral amoxicillin-clavulanate (1-month therapy) is described. This article focuses on diffusion and clinical effect of extended-spectrum beta-lactamases-producing Proteus mirabilis on treatment of gram negative lower extremity infections.

Background And Objectives: The β-lactam/β-lactamase inhibitor combinations ampicillin-sulbactam (SAM) and amoxicillin-clavulanate (AMC) are frequently used to treat Enterobacterales infections and are often assumed to be interchangeable, leading some clinical microbiology laboratories to report antimicrobial susceptibility testing (AST) results for only one of these agents. Given differences in β-lactamase inhibition between sulbactam and clavulanate, we hypothesized that the activities of SAM and AMC may differ.
Methods: To understand the prevalence of discordant SAM and AMC susceptibility results in Enterobacterales species, we analysed AST results obtained by broth microdilution (MicroScan WalkAway, Beckman Coulter) for Enterobacterales isolates recovered from clinical specimens between 2018 and 2022 at an academic medical centre in New York City.

We report the development of a cofactor-free CO fixation platform based on a three-enzyme cascade comprising ferulic acid decarboxylase (AnFDC), phenylalanine ammonia-lyase (AvPAL), and l-amino acid deaminase (PmLAAD). Unlike canonical ATP- or NADPH-dependent CO assimilation pathways, this system uses a prFMN-dependent carboxylation mechanism, enabling efficient CO incorporation under ambient conditions without energy-intensive cofactors. Systematic screening identified AnFDC as the optimal decarboxylase for styrene carboxylation, while AvPAL and PmLAAD were selected for their superior catalytic efficiencies in the cascade.

The article presents the results of the study of polyvalent pyobacteriophage in the complex therapy of nosocomial sinusitis in 30 patients of intensive care units of a multidisciplinary hospital in Moscow. The etiologic factors of nosocomial sinusitis manifestation were the following microorganisms: According to the results of the study it was revealed that at the empirical phage therapy the regression of the main clinical symptomatology was achieved in 90% of patients. The average number of days of this therapy before registration of clinical sanation of the maxillary sinus was 4±1.
