Biomarkers predict non-relapse mortality (NRM) primarily driven by acute graft-versus-host disease (aGVHD) after allogeneic haematopoietic stem cell transplantation (allo-HSCT), yet existing single time point models lack clinical adaptability. In a prospective observational multicentre Chinese cohort of 619 allo-HSCT patients, serum concentrations of suppressor of tumorigenesis 2 (ST2), regenerating islet-derived 3α, tumour necrosis factor receptor 1 (TNFR1), interleukin-6 and interleukin-8 were measured at multiple time points during the first 3 months post-transplantation. Cohorts were divided into modelling (n = 460, training/test) and validation (n = 159) sets.

Background: Chimeric antigen receptor (CAR) T-cell therapy has demonstrated high initial complete remission (CR) rates in B-cell acute lymphoblastic leukemia (B-ALL) patients, including those who relapsed after transplant. However, the duration of remission requires improvements. Whether bridging to a second allogeneic hematopoietic stem cell transplant (allo-HSCT) after CAR-T therapy can improve long-term survival remains controversial.

Article Synopsis
  • - The study analyzed outcomes for 240 pediatric patients with severe aplastic anemia (SAA) who underwent different types of hematopoietic stem cell transplants (HSCT) between 2012 and 2020, revealing that those receiving umbilical cord blood (UCBD) had poorer results compared to matched unrelated donors (MUD) and haploidentical donors (HID).
  • - Patients in the UCBD cohort experienced a higher incidence of graft failure (GF) and had lower rates of platelet engraftment within 180 days post-transplant, taking longer on average to achieve engraftment than those in HID and MUD cohorts.
  • - Three-year failure-free survival (FFS) and graft

Prior studies have suggested that for leukemia patients with high-risk features, haplo-identical-hematopoietic stem cell transplantation (HID-HSCT) has a stronger anti-leukemia effect compared with HSCT using an identical sibling donor (ISD-HSCT). However, it is unclear whether an HID-HSC transplant also augments the graft-versus-leukemia (GVL) effect among refractory/relapsed (R/R) acute myeloid leukemia (AML) patients who are not in remission (NR). We conducted a retrospective analysis of 124 R/R AML patients with NR status who underwent HID-HSCT between April 2012 and December 2016 and compared these to 27 R/R AML patients who underwent an ISD-HSCT within the same timeframe.
