Article Abstract

Objective: This study constructed machine learning (ML) models based on 10 different algorithms using the ratio of preoperative and postoperative hematological indicators to predict the risk factors for perioperative Clavien-Dindo grade III/IV complications after anterior resection in rectal cancer patients who underwent neoadjuvant therapy.

Methods: Rectal cancer patients undergoing anterior resection after neoadjuvant therapy at the Xijing Hospital of Digestive Diseases were retrospectively collected from January 2011 to December 2021. Receiver Operating Characteristic (ROC) curves, calibration curve, Decision Curve Analysis (DCA), accuracy, sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV) and F1 score were used to evaluate the model performance. The Shapley Additive exPlanation (SHAP)algorithm was used to interpret the feature importance of the best model.

Results: The dynamic hematological ratio characteristics before and after surgeries were integrated to construct the predictive models and identify the optimal one. Gradient Boosting Machine (GBM) was considered the best-performing model with the area under the test set curve (AUC: 0.939, 95 %CI: 0.905-0.972), accuracy: 0.903, sensitivity: 0.836, specificity: 0.907. The interpretable SHAP algorithm revealed that hemoglobin, red blood cell ratio and serum calcium ratio were the crucial factors influencing the predictions of the model.

Conclusion: GBM was identified as the optimal predictive model. This model, by predicting the occurrence of perioperative Clavien-Dindo III/IV complications in patients who underwent neoadjuvant therapy and anterior resection, has identified the significant role of hemoglobin, red blood cell ratio and serum calcium ratio, providing guidance for clinical practice. The integration of SHAP provides a clear interpretation of the model's predictions.

Slapping automatism in epileptic seizures: a case series.

Front Hum Neurosci

Department of Neurosurgery, Affiliated Ruijin Hospital, Shanghai Jiao Tong University, School of Medicine, Shanghai, China.

Background: Slapping automatism is a type of automatism observed during epileptic seizures, but its underlying electrophysiological mechanisms remain poorly understood. Stereo-electroencephalography (SEEG) provides a unique opportunity to investigate the associated cortical areas with epileptiform discharges during the slapping automatism.

Case Report: We report five cases of drug-resistant epilepsy in which SEEG recordings captured slapping automatism.

Background: Mitral regurgitation (MR) may rarely worsen after transcatheter aortic valve implantation (TAVI) due to mechanical interference from the transcatheter heart valve (THV). Standard surgical approaches in these cases are often challenging due to anatomical constraints. Thus, there is a need for the development of effective alternatives to address this issue.

Aim: This study evaluated the short-term outcomes of low anterior resection for rectal cancer in Japan before and after the COVID-19 pandemic, with a particular focus on the timing of its reclassification within Japan in May 2023.

Methods: Using data from the Japanese National Clinical Database, we analyzed 109 754 low anterior resection cases between January 2018 and December 2023, categorized into pre-pandemic (February 2020 and earlier), pandemic (March 2020-April 2023), and post-pandemic (May 2023 onward) periods. Trends in the number of low anterior resection cases, postoperative intensive care unit utilization, and complications, including anastomotic leakage and pneumonia, were examined.

Background: Robot-assisted surgery has been widely adopted for the treatment of rectal cancer. Preoperative identification of difficult cases is essential, particularly for surgical training and operating room management. This study aimed to identify preoperative risk factors and develop a predictive scoring system for prolonged pelvic operation time in robot-assisted low anterior resection.

BACKGROUND Gastrointestinal mucormycosis is an underrecognized and underreported fungal infection with a high mortality rate. Diagnosis is often confounded by a non-specific constellation of signs and symptoms. We present a case of neutropenic colitis and ileocecal perforation secondary to gastrointestinal mucormycosis.
