With the advent of newer treatments such as new molecular targeted agents and immunotherapies, the model that selects therapeutic doses on the basis of the maximum tolerated dose is no longer relevant. The emergence of tyrosine kinase inhibitor (TKI) therapy has changed the treatment prospects for chronic myeloid leukemia (CML) and prolonged the long-term survival of CML patients. However, long-term exposure to TKIs is accompanied by adverse events, which may lead to disease progression and even death. It can also increase economic pressure on patients and affect their health-related quality of life. In general, dose reduction is feasible and safe for most patients and can reduce the incidence of adverse events while ensuring efficacy, reduce the financial burden on patients and society, improve the quality of life of patients, and also as a prelude to an attempt at treatment-free remission (TFR). This review will classify the dose optimization of all approved TKIs (imatinib, dasatinib, nilotinib, bosutinib, ponatinib, asciminib, radotinib) at different stages of treatment based on clinical trials and real-life studies, including dose optimization prior to attempting TFR. In addition, we briefly describe the application of therapeutic drug monitoring in dose optimization and the potential benefits of dose optimization on health-related quality of life.

Objectives: Patients with connective tissue diseases (CTD) have a high incidence of cardiac involvement, which often presents insidiously and can progress rapidly, making it one of the leading causes of death. Multiparametric cardiovascular magnetic resonance (CMR) provides a comprehensive quantitative evaluation of myocardial injury and is emerging as a valuable tool for detecting cardiac involvement in CTD. This study aims to investigate the correlations between CMR features and serological biomarkers in CTD patients, assess their potential clinical value, and further explore the impact of pre-CMR immunotherapy intensity on CMR-specific parameters, thereby evaluating the role of CMR in the early diagnosis of CTD-related cardiac involvement.

Purpose: Chemotherapy-induced peripheral neuropathy (CIPN) is a common dose-limiting toxicity associated with oxaliplatin-based chemotherapy in gastric cancer patients. Recent studies suggest that high-dose intravenous selenium may exert neuroprotective effects in patients receiving platinum-based chemotherapy.
Methods: This pilot study analyzed patients with stage III gastric adenocarcinoma who underwent gastrectomy between January and December 2024.

Background: Pediatric patients with autoimmune and inflammatory diseases often require immunosuppressive therapy, which increases their susceptibility to infections, including varicella-zoster virus (VZV). While the live attenuated varicella vaccine is contraindicated in most immunocompromised children, the recombinant subunit vaccine, Shingrix, may offer an alternative preventive strategy. However, data on its safety, immunogenicity, and efficacy in pediatric VZV-naïve patients remain limited.
