Article Abstract

Fibroblast activation protein (FAP) is a type II transmembrane serine protease that is highly expressed in cancer-associated fibroblasts (CAFs) but absent in quiescent fibroblasts. Its overexpression is associated with poor prognosis in various cancers and contributes to treatment resistance. In recent years, radiolabeled FAP inhibitors (FAPI) for PET imaging have shown promising clinical value across a range of cancers. Gastrointestinal (GI) malignancies, which often exhibit a desmoplastic reaction with a high density of FAP-expressing CAFs, are particularly well-suited for FAPI PET. Given the limitations of [F]FDG PET in GI cancers, such as low sensitivity in certain histological subtypes and high physiological background uptake, FAPI PET is expected to serve as a complementary method, potentially enhancing both diagnostic accuracy and treatment guidance. This review provides a comprehensive comparison of the clinical applications of FAPI PET and [F]FDG PET in various GI cancers, including their value in diagnosis, staging, and treatment guidance. Additionally, this review summarizes studies on the expanding role of FAPI PET, including its use in assessing treatment response and predicting prognosis, aiming to provide insights into its potential contribution to the improved management of GI malignancies.

We report the case of a 47-year-old woman who presented with left inguinal swelling; the biopsy of which showed high-grade serous adenocarcinoma. 68Ga-FAPI PET/CT revealed a tracer-avid lesion in the left adnexal region and an enlarged left inguinal nodal mass (site of biopsy). Multiple focal lesions were also seen at the hepatic dome, along the falciform ligament and at the right lateral abdominal wall, suspicious for peritoneal/metastatic deposits.

Breast cancer is one of the most prevalent neoplasms worldwide, with molecular subtypes that influence prognosis and therapeutic strategies. PET/CT with different radiopharmaceuticals has revolutionized diagnosis, staging, and treatment monitoring. [F]-Fluorodeoxyglucose remains the most widely used radiotracer, but it has limitations in certain subtypes, such as invasive lobular carcinoma, where 16α-[F] fluoro-17β-estradiol and [Ga]-FAPI (fibroblast activation protein inhibitors) have demonstrated greater utility.

A 70-year-old man presented with progressive dysuria. 18F-FDG PET/CT showed no abnormal metabolic activity. 68Ga-FAPI PET/CT revealed intense 68Ga-FAPI in the lower left ureter and the left rib, mimicking malignancy with bone metastasis.
