Article Abstract

Introduction: Sepsis is a life-threatening condition characterized by dysregulated inflammatory responses, often leading to multiple organ dysfunction and high mortality rates. Continuous renal replacement therapy (CRRT) and extracorporeal blood purification therapies have emerged as vital adjuncts to manage fluid overload and modulate immune responses in septic shock. This study evaluates the impact of daily fluid balance variation on 90-day mortality and hospital length of stay in ICU patients undergoing CRRT for septic shock with endotoxin activity.

Methods: A post hoc analysis of the EUPHAS 2 project was conducted on 61 ICU patients with nonresponsive endotoxin shock at San Bortolo Hospital (2016-2021). Bayesian joint models assessed the relationship between fluid balance and mortality. Mediation analysis explored the impact of CRRT on mechanical ventilation duration and hospital stay.

Results: Fluid overload and excessive ultrafiltration were associated with prolonged mechanical ventilation and extended hospital stays. Targeted fluid balance management reduced 90-day mortality risk by 50%. CRRT reduced hospital length of stay directly by 5.31 days but indirectly extended it by 11.78 days due to mechanical ventilation. Optimal fluid balance was critical for minimizing mortality and complications.

Conclusions: Careful and tailored fluid management in CRRT is essential for improving survival rates and clinical outcomes in septic shock patients. Continuous monitoring of fluid dynamics is necessary to optimize hemodynamic stability and avoid complications. Future multicenter studies are needed to validate these findings and refine fluid management protocols.

Introduction: This study aimed to evaluate and compare the inflammatory responses of two cordless gingival retraction systems by assessing the levels of interleukin-1 beta (IL-1β) and tumor necrosis factor-alpha (TNF-α) in gingival crevicular fluid (GCF). The objectives were to measure IL-1β and TNF-α levels before and after retraction using kaolin-based and polyvinyl acetate-based cordless gingival retraction materials at baseline, 1 h, and 24 h, and to compare the inflammatory profiles of these materials.

Peritoneal Dialysis (PD) requires a healthy and functional peritoneal membrane for adequate ultrafiltration and fluid balance, making it a vital treatment for patients with end-stage renal disease (ESRD). The spectrum of PD-associated peritoneal fibrosis encompasses a diverse range of collective mechanisms: peritoneal fibrogenesis, epithelial to mesenchymal transition (EMT), peritonitis, angiogenesis, sub-mesothelial immune cells infiltration, and collagen deposition in the sub-mesothelial compact zone of the membrane that accompany deteriorating membrane function. In this narrative review, we summarize the repertoire of current knowledge about the structure, function, and pathophysiology of the peritoneal membrane, focusing on biomolecular mechanisms and signalling pathways that potentiate the development and progression of peritoneal fibrosis.

Far-UVC systems and air cleaners are effective strategies for controlling airborne pathogen transmission, particularly in densely occupied spaces with insufficient ventilation, such as school classrooms. This study evaluates the disinfection performance and ozone (O) formation of different far-UVC systems and air cleaners in a standard-sized classroom using computational fluid dynamics (CFD) simulation. Results show that ceiling-mounted far-UVC systems reduce airborne pathogen exposure by up to 30 % more than upper-room and wall-mounted configurations, based on intake fractions and room-average concentrations.
