Article Abstract

Objective: The aim of this study was to examine the predictive value of the lung ultrasound score (LUS) for successful extubation in preterm infants born at ≤25 weeks.

Methods: This was a single-center, prospective cohort study. Preterm infants with gestational age (GA) ≤ 25 weeks who received invasive mechanical ventilation (IMV) for ≥72 h were included. Lung ultrasound was performed every day. Multivariate logistic regression analysis was used to evaluate factors that predict extubation outcomes.

Results: Ninety-three infants with GA ≤ 25 weeks were included. The mean GA was 24.5 ± 1.2 weeks. Extubation failure occurred in 55 (59.1%) neonates, and success occurred in 38 (40.9%) neonates. The LUS was significantly lower in the successful group than in the failed group (24.0 ± 2.5 vs. 32.1 ± 3.1 p < 0.001). Logistic regression analysis showed that LUS was an independent predictor of successful extubation (odd ratio 0.15 [95% CI 0.045-0.508], P = 0.002). The area under the receiver operating characteristic curve was 0.98 (p < 0.001) for LUS, and a cutoff value of ≥ 28 had 94.6% sensitivity and 92.7% specificity in detecting extubation failure.

Conclusion: The LUS has good accuracy for predicting successful extubation in extremely preterm infants with GA ≤ 25 weeks.

Background: Mucormycosis is a rare, rapidly progressive fungal infection with a high mortality rate. However, clinical data of mucormycosis patients, especially those related to adverse outcomes in China, remain limited.

Objective: To enhance understanding of the clinical characteristics of different infection site mucormycosis and identify the factors associated with adverse outcomes.

Baricitinib Combination Therapy Demonstrates Significant Improvement in Cardiac Conduction Defects in Rapidly Progressive Systemic Sclerosis: A Case Report.

Open Access Rheumatol

August 2025

Department of Rheumatology and Immunology, the First Affiliated Hospital, Jinan University, Guangzhou, 510632, People's Republic of China.

Objective: To evaluate the efficacy of baricitinib in combination therapy for managing refractory, rapidly progressive systemic sclerosis (SSc) with severe cardiac conduction defects and interstitial lung disease (ILD).

Methods: A 48-year-old male patient with SSc complicated by significant cardiac enlargement, third-degree atrioventricular block, heart failure, progressive ILD, and partial intestinal obstruction was included in the study. Prior treatments with mycophenolate mofetil (MMF), tacrolimus, and cyclophosphamide (CTX) had shown limited efficacy.

Malignant phyllodes tumors of the breast are rare fibroepithelial neoplasms with aggressive behavior and high recurrence rates. They pose significant diagnostic and therapeutic challenges due to their overlap with other malignancies, necessitating accurate diagnosis and a tailored treatment approach to improve patient outcomes. A 29-year-old Asian female initially underwent a lumpectomy for a right breast mass diagnosed as a phyllodes tumor on histopathology.

Cor triatriatum is a rare congenital heart defect that divides the right or left atrium into three chambers. Although the diagnosis is typically made within the first years of birth, it can occasionally be made later in adulthood and is frequently associated with other cardiac defects but may be present in isolation. Clinical manifestations range from lung congestion, exhaustion, coughing, and dyspnea to the onset of heart failure.
