Background: Disease progression within 24 months (POD24) significantly impacts overall survival (OS) in patients with follicular lymphoma (FL). This study aimed to develop a robust predictive model, FLIPI-C, using a machine learning approach to identify FL patients at high risk of POD24.
Methods: A cohort of 1,938 FL patients (FL1-3a) from seventeen centers nationwide in China was randomly divided into training and internal validation sets (2:1 ratio). XGBoost was utilized to construct the POD24-predicting model, which was internally validated in the validation set and externally validated in the GALLIUM cohort. Key predictors of POD24 included lymphocyte-to-monocyte ratio (LMR), lactate dehydrogenase (LDH) > ULN, low hemoglobin (Hb), elevated beta-2 microglobulin (β2-MG), maximum standardized uptake value (SUVmax), and lymph node involvement. The FLIPI-C model assigned 2 points to LMR and 1 point to each of the other variables.
Results: The FLIPI-C model demonstrated superior accuracy (AUC) for predicting POD24 and 3-year overall survival (OS) in both the internal (AUC POD24: 0.764, OS: 0.700) and external validation cohorts (AUC POD24: 0.703, OS: 0.653), compared to existing models (FLIPI, FLIPI-2, PRIMA-PI, FLEX). Decision curve analysis confirmed the superior net benefits of FLIPI-C.
Conclusions: Developed using a machine learning approach, the FLIPI-C model offers superior predictive accuracy and utilizes simple, widely available markers. It holds promise for informing treatment decisions and prognostic assessments in clinical practice for FL patients at high risk of POD24.

Background: T follicular helper (TFH) cell lymphoma is complex, and we hope to provide a new perspective for its diagnosis.
Methods: We analysed the immunophenotypes of 89 mature T-cell lymphomas, including 52 nodal lymphomas of TFH origin, as well as 32 benign lymph node samples and 30 healthy bone marrow samples, by flow cytometry (FCM).
Results: Among pan-T cell markers, CD4CD5CD3 is the typical pattern that distinguishes TFH lymphoma from other T-cell lymphomas.
Surgical pathologists who examine gastric biopsies must triage exuberant lymphoid infiltrates for hematopathology consultation, a task that should account for resource and time utilization. We assembled all cases of chronic gastritis sent by surgical pathologists to hematopathology due to concern for low-grade lymphoma over a 4-year interval. The cases were ultimately classified as reactive (n=37), atypical (n=9), or lymphoma (n=18).

Epcoritamab is a subcutaneous CD3xCD20 bispecific antibody approved as monotherapy for relapsed/refractory (R/R) follicular lymphoma (FL). We evaluated fixed-duration epcoritamab with rituximab plus lenalidomide (R2) in R/R FL in arm 2 of EPCORE® NHL-2 (phase 1b/2; NCT04663347). Patients received epcoritamab (2 step-up doses, then 48-mg full doses) for up to 2 years and R2 for up to 12 cycles (28 days/cycle).

Obinutuzumab is a humanized type II anti-CD20 monoclonal antibody that is widely used in B-cell lymphomas including follicular lymphoma (FL) and chronic lymphocytic leukemia (CLL). Multiple clinical studies have shown that compared with rituximab combined with chemotherapy, obinutuzumab combined with chemotherapy can significantly improve the progression-free survival (PFS) of patients, effectively reduce the risk of disease progression, and improve patient prognosis. The main adverse effects of obinutuzumab include infusion reactions, myelosuppression, infection, cardiotoxicity, tumor lysis syndrome (TLS), etc.

An 84-year-old man with a history of amputation and follicular lymphoma developed a non-healing ulcer on his stump, initially diagnosed as a pressure ulcer cause by the clinic and lack of B-symptoms. Despite wound care, the lesion worsened. A biopsy revealed de novo diffuse large B-cell lymphoma (DLBCL), non-germinal center subtype.
