Background: Arrhythmogenic cardiomyopathy (ACM) is characterized by fibrofatty myocardial replacement and increased arrhythmic risk. Although exercise exacerbates desmosomal ACM, the prognostic significance of arrhythmias during exercise stress tests (ESTs) remains unclear.
Objectives: The goal of this study was to determine the impact of ventricular arrhythmia observed during peak exercise and/or recovery EST phases on the risk of major ventricular arrhythmia (MVA) events in patients with desmosomal ACM.

S-palmitoylation has emerged as a critical integrator of lipid overload and cardiovascular dysfunction. Disordered lipid metabolism inundates endothelial cells, vascular smooth muscle cells and macrophages with triglyceriderich lipoproteins, oxidized LDL and saturated fatty acids, expanding the intracellular palmitoylCoA pool and perturbing redox balance. Protein Spalmitoylation, the reversible attachment of palmitate to cysteine residues, converts excess palmitoylCoA into broad alterations in signalling and membrane dynamics.

The widespread use of cardiac magnetic resonance imaging (MRI) in clinical practice has enabled the identification of numerous patients with evident damage from previous myocarditis, whether known or unknown. For years, myocardial fibrosis has been a topic of interest due to its established correlation with arrhythmic events in various clinical settings, including ischemic heart disease, dilated cardiomyopathy, and hypertrophic cardiomyopathy. MIBG scintigraphy is a method widely used in patients who are candidates for defibrillator implantation or have experienced heart failure.

Atrial fibrillation (AF), the most prevalent sustained cardiac arrhythmia, poses a significant burden on global morbidity and healthcare expenditure. Although endocardial catheter ablation and surgical ablation are established therapeutic strategies, each exhibits inherent limitations in achieving comprehensive substrate modification. Hybrid ablation therapy, integrating both endocardial and epicardial approaches, aims to overcome these limitations by enabling the more extensive and transmural targeting of arrhythmogenic foci and the complex atrial substrate.

Background: Mitral valve prolapse (MVP) in some cases may predispose to life-threatening ventricular arrhythmias. The aims of this study were to evaluate determinants of arrhythmic MVP (AMVP) in a large population and to identify variables associated with sudden cardiac arrest (SCA) or sudden cardiac death.
Methods: This ambispective cohort study included patients 18 to 70 years of age with MVP with preserved ejection fraction and no coronary artery disease, cardiomyopathies, or channelopathies.
