Many different risk stratification systems have been formulated for thyroid nodules, differing in their fine-needle aspiration cytology (FNAC) indication, suggesting a lack of consensus around the world. This prospective study was conducted to find the best guideline for risk stratification, for a better malignancy yield, and with reduced rates of negative FNACs among three Thyroid Imaging, Reporting, and Data System (TIRADS) guidelines. A total of 625 thyroid nodules with conclusive FNAC or histopathological diagnosis were included in the study. Various sonographic parameters were recorded. They were classified into categories as per the three guidelines and compared with FNAC diagnosis. The guidelines were evaluated in terms of sensitivity, specificity, predictive values, and diagnostic accuracy. Sensitivity and specificity were compared by McNemar's test. American College of Radiology (ACR) TIRADS had the highest diagnostic accuracy (56.8%), specificity (50.75%), positive predictive value (23.92%), lowest rates of negative FNACs (76.08%), and high negative predictive value (97.84 %). Korean (K) TIRADS had the maximum sensitivity (97.75%), highest negative predictive value (98.44%), and gross malignancy yield. European TIRADS was between the two other guidelines in most parameters with specificity like K TIRADS. All the three guidelines are very good screening tools, with comparable high sensitivity. ACR TIRADS is better in terms of specificity and reduced rates of negative FNACs. Including the presence of a suspicious cervical lymph node as a criterion and more frequent follow-up might further improve the diagnostic performance of the guideline.

Objective: To explore the value of microflow patterns based on superb microvascular imaging (SMI) combined with greyscale ultrasound in thyroid nodule diagnosis and biopsy recommendation.
Materials And Methods: Adult patients with thyroid nodules were recruited from May 2023 to February 2024. The greyscale features of nodules were evaluated according to the five ultrasound risk stratification systems (RSSs).
Introduction We aimed to investigate malignancy rates in atypia of undetermined significance (AUS) subcategories and their association with cytomorphologic and ultrasonographic features. Methods A total of 201 thyroid nodules with AUS cytology that underwent surgical resection were analy-zed, including 169 AUS-Nuclear (AUS-N) and 32 AUS-Other (AUS-O) nodules. Cytomorphological and ultrasonographic features, along with the American College of Radiology Thyroid Imaging Reporting and Data System (ACR-TIRADS) and the European Thyroid Imaging Reporting and Data System (EU-TIRADS) classifications, were analyzed to assess the association between malignancy and AUS subcategories.

Purpose: This study aimed to evaluate and compare the diagnostic performance of various Thyroid Imaging Reporting and Data Systems (TIRADS), with a particular focus on the artificial intelligence-based TIRADS (AI-TIRADS), in characterizing thyroid nodules.
Methods: In this retrospective study conducted between April 2016 and May 2022, 1,322 thyroid nodules from 1,139 patients with confirmed cytopathological diagnoses were included. Each nodule was assessed using TIRADS classifications defined by the American College of Radiology (ACR-TIRADS), the American Thyroid Association (ATA-TIRADS), the European Thyroid Association (EU-TIRADS), the Korean Thyroid Association (K-TIRADS), and the AI-TIRADS.

This study aimed to compare: the performance of K-TIRADS, EU-TIRADS and ACR TIRADS when used by observers with different levels of experience compared with the gold standard of cytology, and to evaluate the diagnostic performance of CAD (computer-aided design) compared with TI-RADS systems. In total, 323 thyroid nodules were evaluated in patients who were candidates for needle aspiration. Three observers with different levels of experience evaluated the diagnostic accuracy of three risk stratification systems (ACR TI-RADS, EU-TIRADS and K-TIRADS) and CAD software (S-Detect, made by Samsung) in characterizing the nodules.
