The self-proclaimed first publicly available dataset of Monkeypox skin images consists of medically irrelevant images extracted from Google and photography repositories through a process denominated web-scrapping. Yet, this did not stop other researchers from employing it to build Machine Learning (ML) solutions aimed at computer-aided diagnosis of Monkeypox and other viral infections presenting skin lesions. Neither did it stop the reviewers or editors from publishing these subsequent works in peer-reviewed journals. Several of these works claimed extraordinary performance in the classification of Monkeypox, Chickenpox and Measles, employing ML and the aforementioned dataset. In this work, we analyse the initiator work that has catalysed the development of several ML solutions, and whose popularity is continuing to grow. Further, we provide a rebuttal experiment that showcases the risks of such methodologies, proving that the ML solutions do not necessarily obtain their performance from the features relevant to the diseases at issue.

Methods: We conducted a prospective cohort study of individuals diagnosed with mpox in Toronto, Canada. Skin lesion swabs were collected weekly to quantify infectious monkeypox virus (MPXV) shedding through cell culture.

Mpox, caused by the monkeypox virus (MPXV), re-emerged as a significant global health issue in 2024, resulting in the declaration of a second Public Health Emergency of International Concern (PHEIC). The emergence of Clade Ib in Central Africa, particularly affecting children and immunocompromised individuals, and the ongoing global spread of Clade IIb have raised concerns about increased transmissibility and virulence, potentially driven by accelerated APOBEC3-mediated viral mutations. Novel vaccination strategies include the licensed JYNNEOS and Japan's LC16 KMB, as well as promising mRNA vaccine candidates currently in clinical trials.

The recent surge in global monkeypox (mpox) outbreaks highlights the critical need for developing antiviral agents targeting orthopoxvirus infections. Ibrutinib, a selective Bruton tyrosine kinase (BTK) inhibitor initially approved by the FDA in 2013 for treating chronic lymphocytic leukemia (CLL) and mantle cell lymphoma (MCL), has emerged as a potential antiviral candidate based on a high-throughput screening for its efficacy against vaccinia virus (VACV). This study evaluated the antiviral capability of ibrutinib against VACV, mpox virus (MPXV), and lumpy skin disease virus (LSDV), demonstrating its strong antiviral activity against multiple poxviruses.

Mpox, caused by monkeypox virus (MPXV) infection, has emerged as a significant global health threat. The World Health Organization (WHO) has twice declared a Public Health Emergency of International Concern for mpox: first for the 2022-2023 global outbreak and subsequently for concurrent outbreaks in Africa. Beyond MPXV, other members of the Orthopoxvirus genus also pose growing risks of zoonotic spillover, with the potential to jump from animal reservoirs to humans.
