Article Abstract

Objectives: The purpose of this pictorial review is to present the imaging spectrum of renal oncocytomas with radiological-pathological correlation.

Conclusion: The differences in tumour cellularity (high cellularity or low cellularity with abundant stroma) and haemorrhagic/cystic change contribute to a wide spectrum of imaging findings of renal oncocytomas. Imaging findings substantially overlap those of common subtypes of clear cell and non-clear cell renal cell carcinomas. Multifocal renal oncocytomas are not rare, and making the diagnosis of oncocytoma with concomitant renal cell carcinoma is difficult. In addition, renal oncocytomas that demonstrate interval growth or develop in the setting of end-stage renal disease may be mistaken for malignancy.

Teaching Points: • High cellular components demonstrate avid arterial enhancement and subsequent washout. • Low cellular components demonstrate gradual subsequent enhancement owing to abundant stroma. • Cystic and hemorrhagic changes may account for lesion heterogeneity in the delayed phase. • Multifocal oncocytomas and oncocytomas coexisting with renal cell carcinoma are not rare. • Renal oncocytomas may demonstrate interval growth.

Renal oncocytomas are benign renal tumours characterized by a central stellate scar that are indistinguishable on CT/MR imaging from malignant chromophobe renal cell carcinomas (ChrRCCs). Renal oncocytomas and ChrRCCs can be separate entities but can also co-exist on a spectrum in hybrid oncocytic/ chromophobe tumours. In the past, invasive biopsy and pathologic diagnosis has been relied on to differentiate these lesion and direct management.

Objective: To evaluate the expression of trophoblast cell surface antigen-2 (TROP-2), a broadly expressed antibody-drug conjugate (ADC) target, in non-clear cell renal cell carcinoma (nccRCC), and to perform a proof-of-concept analysis assessing the cytotoxic efficacy of the TROP-2-directed ADC Sacituzumab govitecan (SG) in RCC cell lines.

Methods: A cohort comprising clear cell RCC (ccRCC,  = 44), papillary (pRCC,  = 22), chromophobe (chRCC,  = 22), and benign renal tumors ( = 8, including oncocytoma and angiomyolipoma) was analysed using reverse transcription quantitative PCR (RT-qPCR), immunohistochemistry (IHC) with H-score quantification, and enzyme-linked immunosorbent assay (ELISA). In RCC cell lines, TROP-2 protein levels were assessed by Western blotting and flow cytometry, and SG cytotoxicity was evaluated using MTT assays.

Peroxidasin (PXDN), is a heme peroxidase with a critical role in the crosslinking of type IV collagen, being essential for basement membrane integrity. Overexpression of PXDN has been associated with poor clinical outcomes in many cancers; however, little is currently known about its role in renal cell carcinoma (RCC). In this study, we characterized the expression of PXDN in tumour tissue and adjacent non-neoplastic tissue from cases of clear cell renal cell carcinoma (ccRCC), chromophobe RCC (chRCC), and renal oncocytoma using immunohistochemistry.
