Objective: To demonstrate that the feasibility of using the da Vinci robotic XI surgical system for breast reconstruction with pectoralis major fascia instead of latissimus dorsi flaps.

Methods: A retrospective analysis was conducted on the clinical data of 33 female patients with breast cancer who were treated with robotic nipple sparing mastectomy and immediate breast reconstruction with gel implant (RNSMIBR) between September 2022 and June 2024 and met the selection criteria. The surgical techniques employed included the use of a latissimus dorsi muscle flap (LDMF) in seven cases (Group A), a LDMF without skin island in nine cases (Group B) and a pectoralis major fascia in 17 cases (Group C).

During robotic nipple-sparing mastectomy, the breasts were covered with gauze cooled by ice water to reduce skin temperature in 99 cases (group A) and were not treated in 29 cases (group B).

Mild encephalopathy with reversible splenial lesion (MERS) is a rare clinical-radiological syndrome with a favorable prognosis that typically presents with central nervous system symptoms such as altered mental status, delirious behavior, seizures, muscle weakness, ophthalmoplegia, and headache. The diagnosis of MERS is based on a constellation of central nervous system symptoms within one week of fever, a lesion in the splenium of the corpus callosum, and complete resolution without sequelae. Both clinical and imaging findings generally resolve within a few months.

Aim: To this date, no single treatment method provides pathological correction of the mechanisms associated with tissue reorganization observed in later-stage breast cancer-related lymphedema (BCRL).

Methods: To define a personalized approach to the management of patients with iatrogenic lymphedema, we performed a systematic review of literature without a comprehensive meta-analysis to outline existing molecular- genetic patterns, overview current treatment methods and their efficacy, and highlight the specific tissue-associated changes in BCRL conditions and other bio-engineering approaches to develop personalized therapy.
