Background: Aging is increasingly recognized as a modifiable biological process influenced by genetic, environmental, and lifestyle factors. Recent advances in regenerative medicine and artificial intelligence (AI) have reshaped the field of cosmetic dermatology, shifting the focus from temporary aesthetic improvements to long-term interventions aimed at preserving skin vitality and longevity.
Aim: This narrative review aims to synthesize emerging knowledge from 2010 to 2025 on the integration of regenerative strategies, biological modulators, immunologic regulation, microbiome modulation, and AI-driven personalization in the context of aesthetic longevity.
Introduction: Treatments such as 4% 5-fluorouracil (5-FU) cream have demonstrated strong efficacy in lesion clearance of actinic keratosis; however, local skin reactions (LSR) during treatment remain a significant challenge, potentially affecting patient adherence.
Objective: We sought to build consensus on management of LSR associated with 4% 5-FU using the Delphi methodology.
Methods: Twenty-eight expert dermatologists participated in a 3-round Delphi process.
The bidirectional communication between the skin and brain has emerged as a promising scientific paradigm in dermatology and cosmetic medicine. Neurocosmetics, a frontier at the intersection of neuroscience, dermatology, and psychodermatology, aims to target this skin-brain axis to enhance not only skin health but also emotional well being. This commentary explores the underlying neurocutaneous and neuroimmune mechanisms, the emerging role of the skin microbiome in emotion-linked skin responses, and how artificial intelligence technologies can help personalize these strategies.

Background And Purpose: In patients with metastatic melanoma who respond to anti-PD-1 therapy, the proliferation of intra-tumour CD8+ T cells is directly correlated with the clinical response, making tumour-infiltrating lymphocytes (TILs) a treatment of interest in combination with a PD-1 inhibitor, which is the undisputed gold standard in the management of metastatic melanoma.
