Although the precise neurobiological mechanisms underlying its antidepressant actions are not fully determined, a critical feature of ketamine's clinical efficacy may be its antianhedonic action. Anhedonia is an endophenotype of depression defined by decreased responsivity to previously rewarding stimuli and is generally not ameliorated by conventional antidepressants, emphasizing the need to examine underlying behavioral mechanisms of action.

Rationale: Modafinil has been proposed as a potentially effective clinical treatment for neuropsychiatric disorders characterized by cognitive control deficits. However, the precise effects of modafinil, particularly on brain network functions, are not completely understood.
Objectives: To address this gap, we examined the effects of modafinil on resting-state brain activity in 30 healthy adults using microstate analysis.
Despite the prominence of anhedonic symptoms associated with diverse neuropsychiatric conditions, there are currently no approved therapeutics designed to attenuate the loss of responsivity to previously rewarding stimuli. However, the search for improved treatment options for anhedonia has been reinvigorated by a recent reconceptualization of the very construct of anhedonia, including within the Research Domain Criteria (RDoC) initiative. This chapter will focus on the RDoC Positive Valence Systems construct of reward learning generally and sub-construct of probabilistic reinforcement learning specifically.
