Vascular surgical site infections (SSI) are common and associated with graft infection, surgical reintervention, and increased lengths of stay. While antibiotic prophylaxis and negative pressure dressings have improved SSI rates, reported incidence remains as high as 30%. Robotic approaches have decreased surgical site infections in multiple surgical specialties, but remain without a vascular surgery indication.

Background: The utilization of robot-assisted laparoscopic pyeloplasty (RALP) for the surgical correction of ureteropelvic junction obstruction (UPJO) continues to increase. The aim of this study was to determine whether robot-assisted laparoscopic pyeloplasty (RALP) is a safe and equally efficacious procedure compared to open pyeloplasty (OP) in infants ≤12 months old.
Methods: All patients ≤12 months old who underwent RALP or OP between January 2012 and January 2021 at five participating centers were included in this study.

Background: Familial cerebral cavernous malformations (FCCM), Sturge-Weber Syndrome (SWS), and hereditary hemorrhagic telangiectasia (HHT) are driven by genetic mutations causing varying vascular dysmorphism and risk of brain bleeding. Cerebral microbleeds (CMBs) are associated with the aging process with less characterized genetic drivers. This study hypothesizes that common and distinct circulating microRNAs (miRNAs) can reflect mechanisms of vascular dysmorphism and bleeding, which can serve as potential biomarkers in clinical contexts.

Monosomy 7 (-7) and deletions of chromosome arm 7q (del(7q)) are prevalent high-risk cytogenetic abnormalities that often co-occur with del(17p) (harboring ). To identify novel targeted therapies based on specific vulnerabilities in high-risk myeloid malignancies, we investigated druggable, chromosome 7-encoded essential genes that are monoallelically deleted in the context of -7/del(7q), that is, collateral lethal genes. Background: In the US, donor hearts for transplant are currently allocated to the candidates with the highest risk of death on the waiting list, based on a categorical status-based system. The upcoming continuous distribution system provides an opportunity to implement a post-transplant risk score that may help avoid futile transplants.
Methods: In this observational study of the Scientific Registry of Transplant Recipients (SRTR), a novel US transplant risk score (US-TRS) was developed and validated using a mixed-effects Cox proportional hazards model.

Spina bifida is a birth defect resulting from abnormal embryonic development of the neural tube. Though spina bifida is divided into several subtypes, myelomeningocele-the most severe form of spina bifida often associated with a markedly diminished quality of life-accounts for a significant portion of cases. A broad range of genetic and environmental factors, many of which are still unknown, influence spina bifida, making it difficult to provide a comprehensive etiology for the disorder.

Carotid artery stenosis in patients with complex comorbidities, severe calcification, and prior surgical interventions presents significant procedural challenges. Traditional access routes for stenting, such as transfemoral or transcarotid, may be unfeasible in such cases, requiring innovative alternatives. A 79-year-old man with symptomatic high-grade left internal carotid artery stenosis, severe aortoiliac occlusive disease, and a history of failed transfemoral stenting was treated successfully with left internal carotid artery stenting via right brachial artery access.

We investigated the prognostic impact of blast counts, allelic state determinants (number of hits, del(17p), variant allele frequency, complex karyotype),and a novel karyotypic clonal cell fraction (≤50% clonal cells vs >50% clonal cells (termed 'CK50')) in 495 individuals with mutated ( ) myeloid neoplasms. Outcome examined was 24-month survival (OS24). The cohort (median age 71) included 29% (144/495) myelodysplastic syndromes (MDS)/MDS-acute myeloid leukaemia (AML) (1%-19% blasts) and 71% (351/495) AML (≥20% blasts), with 18% (81/460) having low CK50.

Background: Histological transformation to small-cell lung cancer (SCLC) is an under-recognized but clinically relevant mechanism of resistance in epidermal growth factor receptor (EGFR)-mutant non-small-cell lung cancer (NSCLC). While platinum-based chemotherapy (CT) has emerged as standard treatment after small-cell transformation, optimal treatment strategies are not defined, particularly with regard to the role of immune checkpoint inhibitors (ICIs) or continuation of EGFR-tyrosine kinase inhibitors (TKIs).
Materials And Methods: We conducted an international, retrospective, observational study on patients with EGFR-mutated NSCLC that transformed to SCLC, focusing on treatment and outcomes.
