Background: Gynecologic laparotomy surgery often leads to moderate to severe pain. Effective analgesia is essential for patient postoperative recovery. We aimed to evaluate the effect of low-dose esketamine on quality of recovery when used as part of multimodal analgesia in patients undergoing open gynecologic surgery.
Methods: 140 female patients undergoing gynecologic laparotomy under general anesthesia were randomly assigned to receive intraoperative infusion of either 0.25 mg/kg/h esketamine (study group) or an equivalent volume of normal saline (control group). The primary outcome was quality of recovery at 24 h postoperatively, assessed using the 40-item quality of recovery questionnaire (QoR-40). Secondary outcomes included morphine consumption, pain scores, functional outcomes, adverse events, postoperative depressive symptom, chronic pain, and health-related quality of life.
Results: The global QoR-40 score at 24 h was not significantly different between the two groups [study group, 178 (165-186); control group, 175 (164-186); median difference, 2; 95% CI, -3 to 7; = 0.467). Secondary outcomes were comparable between the two groups. More patients in the study group had extubation time ≥ 10min (15.7% vs. 2.9%; = 0.009) and were over sedated (25.7 vs. 4.3%, < 0.001) during post-anesthesia care unit stay.
Conclusions: Intraoperative infusion of 0.25 mg/kg/h esketamine had no additional benefit on early quality of recovery when used as part of multimodal analgesic regimen in patients undergoing open gynecologic surgery. Future investigations are needed to determine the optimal dosing regimen and identify subgroups of gynecologic patients who would benefit most from esketamine.

A method for determination of ten kinds of sweeteners in soybean products by multi-plug filtration cleanup (-PFC) combined with ultra-high-performance liquid chromatography-tandem mass spectrometry (UPLC-MS/MS) was established. The sample was extracted with acetonitrile (containing 1% formic acid), degreased by using -hexane liquid-liquid extraction and purified by solid phase extraction using an -PFC column (Oasis PRiME HLB). The analytes were separated by using a Waters ACQUITY UPLC® BEH C (2.

Acute ischemic stroke (AIS) remains a leading cause of mortality and long-term disability globally, with survivors at high risk of recurrent stroke, cardiovascular events, and post-stroke dementia. Statins, while widely used for their lipid-lowering effects, also possess pleiotropic properties, including anti-inflammatory, endothelial-stabilizing, and neuroprotective actions, which may offer added benefit in AIS management. This article synthesizes emerging evidence on statins' dual mechanisms of action and evaluates their role in reducing recurrence, improving survival, and mitigating cognitive decline.

Background: Powerlifting is a strength sport featuring some of the world's strongest athletes. Recent decades have seen an exponential increase in research into the applied sport science and medicine of powerlifting and its Paralympic counterpart, para powerlifting. A scoping review of the area would provide athletes, coaches, policymakers, and researchers with an overview of the existing evidence to support performance, reduce injury, and foster further growth of these sports.

Introduction And Objectives: High socioeconomic status (SES) is associated with improved oncological outcomes across various cancer types, including prostate cancer. However, limited evidence exists regarding the impact of SES and lifestyle factors on patient-reported outcomes (PROs), including quality of life (QoL), health status (HS), and functional recovery following radical prostatectomy (RP).
Materials And Methods: We conducted a retrospective single-center analysis of 327 patients undergoing RP (177 open, 150 robotic-assisted) assessing pre- and postoperative functional outcomes (QoL, HS, erectile function, continence).

In the United States, approximately 20 million individuals, age 12 and older, have a substance use disorder (SUD), with an average age of first-time use at 13 years old. Evidence has shown that many SUDs begin in adolescence, and involvement with the legal system can ensue. Adolescents with first-time drug and alcohol arrests can be referred to an Adolescent Intervention Program (AIP) as an alternative to jailtime from the juvenile court system.
