Complicated hilar anatomy may be technically challenging for thoracic surgeons, especially during video-assisted thoracic surgery lobectomy. In such instances, to prevent hemorrhagic complications and conversion to open thoracotomy, several methods of proximal and distal vascular control have been proposed. Here, we present a technique of unnecessary occlusion of the main pulmonary artery and preserved pulmonary vein during a challenging thoracoscopic left lower lobectomy in patient with lung cancer and interlobar lymphadenopathy.

BACKGROUND Chest wall hernia and residual pleural space are known complications after thoracoscopic anatomical lung resection. Some risk factors for chest wall hernia have been described; however, residual pleural space has never been described as one of them. We present 2 cases suggesting that postoperative residual air space can represent a newly identified risk factor for chest wall hernia.

Primary pulmonary sarcoma is a rare disease and is much less common than lung cancer among tumors arising from pulmonary cysts. We report the case of a female patient who showed multifocal cysts in the left S9-10. Follow-up computed tomography (CT) revealed that the cyst tended to regress, but the solid component of the cyst wall continued to thicken, growing to a 10-cm-diameter tumor.

Pulmonary epithelial-myoepithelial carcinoma (P-EMC) is a rare type of salivary gland tumour of the lung. Due to its rarity and lack of long-term follow-up data, there is no established standard for optimal treatment or duration of follow-up. The present study reports the case of a 58-year-old female patient with P-EMC originating from the middle part of the bronchus and presenting as an endobronchial mass in the left superior lobe.

Epithelioid hemangioendothelioma (EHE) with pleural involvement presents significant diagnostic challenges, particularly in terms of differentiating it from malignant pleural effusion caused by other types of cancer, such as lung carcinoma. While most cases of EHE follow an indolent course, some can deteriorate rapidly, particularly those with serosal involvement such as pleural metastasis. In this report, we describe a case in which semi-rigid thoracoscopic cryobiopsy under local anesthesia yielded adequate specimens safely for diagnosis of pleural dissemination of EHE.
