Objectives: Disseminated nontuberculous mycobacterial disease (dNTMd) remains a rare but serious complication in people living with HIV (PLWH). This study aimed to assess whether dNTMd independently contributes to delayed CD4-cell recovery following antiretroviral therapy (ART) initiation.
Methods: This retrospective 1:3 single center case-control study analyzed patient data from 2004 to 2023.

A 79-year-old female diagnosed with myelodysplastic syndrome (MDS) and germline GATA2 mutation, on compassionate cobimetinib, was admitted with subacute cough and dyspnea. Chest imaging demonstrated a new, large, left hilar mass and consolidation with scattered diffuse mediastinal, supraclavicular, and hilar lymphadenopathy. A core biopsy of the right supraclavicular lymph node was performed.

Disseminated Mycobacterium avium complex (DMAC) infection is a major AIDS-defining condition with diagnostic challenges due to nonspecific symptoms. This study, for the first time in Iran, aimed to discriminate MAC organisms in 100 HIV-positive patients directly from clinical specimens and assess their clinical significance, epidemiological characteristics, and associated risk factors through a detailed review of medical and demographic records.
