Article Abstract

Background: This study evaluates the association between left atrial and right atrial (LA, RA) parameters and a composite endpoint (CEP) of all-cause death, thromboembolism, acute coronary syndrome, and heart failure hospitalization in atrial fibrillation (AF) patients.

Material And Methods: Patients were prospectively enrolled. At baseline, the following echocardiogram parameters were measured: LA and RA antero-posterior diameter index (iLAAPD, iRAAPD), LA and RA volume index (LAVi, RAVi), LA and RA sphericity index (LASI, RASI), LA and RA emptying fraction.

Results: A total of 489 patients (61.3% males) with a median age of 75 (66-80) years and a median CHA2DS2VASc score of 3 (2-5) were enrolled (92.2% receiving anticoagulation). Permanent AF was present in 40.5% of the total cohort. After a median follow-up of 1114 (392-1384) days, 129 patients (26.3%) reached the CEP. The highest sensitivity for CEP was for LA emptying fraction <28% and iRAAPD >24 mm/m2 (72% and 73%, respectively) while the best negative predictive values were for iLAAPD and LAVi (both 81%). Right atrial parameters were not associated with CEP. Discrimination analysis using net reclassification improvement (NRI) showed that iLAAPD and LAVi significantly improved patient reclassification compared to a null model without atrial parameters (iLAAPD NRI 0.30; P = 0.005; LAVi NRI 0.32; P = 0.002). Multivariate Cox regression analysis found that LA dimensions, volume, and function were associated with a higher risk of adverse outcomes and significantly improved risk prediction for the CEP.

Conclusions: LAVi and iLAAPD enhance discrimination and risk prediction for adverse outcomes in AF patients.

Background: Pulsed electric field ablation (PFA) techniques for treating cardiac arrhythmias have attracted considerable interest. For example, atrial fibrillation can be effectively treated by pulmonary vein isolation using PFA. However, some arrhythmias originate deep within the myocardium, making them difficult to reach with conventional ablation methods.

Background: Atrial fibrillation (AF) and heart failure (HF) frequently coexist in patients, with the development of AF often preceding HF decompensation. We sought to evaluate whether daily remote monitoring of ICD parameters could predict AF occurrence using machine learning techniques in a real-world cohort.

Methods: Data from patients with primary prevention ICDs transmitted daily to the Northwell centralized remote monitoring center between 2012 and 2021 were extracted.

Background: Accurate measurement of echocardiographic parameters is crucial for the diagnosis of cardiovascular disease and tracking of change over time; however, manual assessment requires time-consuming effort and can be imprecise. Artificial intelligence has the potential to reduce clinician burden by automating the time-intensive task of comprehensive measurement of echocardiographic parameters.

Objectives: The purpose of this study was to develop and validate open-sourced deep learning semantic segmentation models for the automated measurement of 18 anatomic and Doppler measurements in echocardiography.

Rationale And Objectives: This study aimed to investigate the role of cardiac magnetic resonance (CMR)-derived left atrial (LA) strain parameters in evaluating early cardiac dysfunction in hypertensive patients and to assess their diagnostic utility for left ventricular diastolic dysfunction (LVDD).

Methods: A total of 150 hypertensive patients and 60 healthy controls were retrospectively enrolled, with all participants undergoing both echocardiographic and CMR examinations. Hypertensive patients were stratified by LVDD severity based on current guidelines.

Background: Interatrial block (IAB) is an electrocardiographic manifestation of atrial conduction delay and structural remodeling. While it has been linked to atrial fibrillation (AF) and thromboembolic events, its potential role in predicting left atrial appendage (LAA) thrombus formation remains underexplored. This study aimed to investigate the association between IAB and the presence of LAA thrombus in patients with nonvalvular AF (NVAF) or atrial flutter referred for rhythm control procedures.
