Exosomes present in the circulatory system demonstrate considerable promise for the diagnosis and treatment of diseases. Nevertheless, the complex nature of blood samples and the prevalence of highly abundant proteins pose a significant obstacle to prompt and effective isolation and functional evaluation of exosomes from blood. Here, we present a fully integrated lab-on-a-disc equipped with two nanofilters, also termed iExoDisc, which facilitates automated isolation of exosomes from 400 μL blood samples within 45 min. By integrating the plasma separation module, highly abundant protein removal module, and nanopore membrane-based total isolation module, the resulting exosomes exhibited significantly increased purity (∼3-6-fold) compared to conventional ultracentrifugation and polymer precipitation. Additionally, we then successfully performed nontargeted and targeted glycan profiling on exosomes derived from clinical triple-negative breast cancer (TNBC) patients using MALDI-TOF-MS and lectin microarray containing 56 kinds of lectins. The findings from both methodologies indicated that galactosylation and sialylation exhibit potential as diagnostic indicators for TNBC. Finally, by utilizing the exosome-specific glycosylated protein CD63 as a proof-of-concept, we successfully realized the integration of point-of-care on-chip exosome separation and in situ detection with 2 h. Thus, the iExoDisc provides a potential approach to early cancer detection, liquid biopsy, and point-of-care diagnosis.

Chronic hyponatremia is increasingly recognized as a potential contributor to impaired bone health, although the underlying pathophysiological mechanisms have not yet been fully elucidated. Experimental studies have demonstrated that low serum sodium levels affect both osteoclast and osteoblast function, resulting primarily in increased bone resorption and secondarily in reduced bone formation. In humans, however, evidence regarding the effects of hyponatremia on bone remains limited.

Background: United States healthcare systems face an imminent need to recruit and retain direct care registered nurses (RNs), driven in part by high nurse turnover rates, which contribute to substantial economic and non-economic burdens. The pandemic exacerbated nurse turnover rates, while colleges of nursing simultaneously experienced a faculty shortage lending to fewer baccalaureate prepared RNs entering the workforce. Our large academic health system experienced similar challenges while the College of Nursing (CON) rapidly increased student enrollment to meet the need for additional RNs requiring an immediate and creative solution.

Background: Targeted bottom-up proteomics is of great interest for the straightforward, accurate, and sensitive measurement of specific protein biomarkers from surrogate peptide fragments. However, this approach typically relies on off-line enzymatic digestion with trypsin, a time-consuming step that may be inadequate for covering certain sequence regions containing important post-translational modifications (PTMs).
Results: In this study, we present an in-line enzymatic digestion strategy for the targeted bottom-up analysis of α-synuclein (α-syn), which is a protein biomarker of Parkinson's disease (PD).
