Article Abstract

Background: Acute-on-chronic liver failure (ACLF) represents the terminal and most lethal phase of acute decompensated cirrhosis. Systemic inflammation plays a critical role in the pathogenesis of ACLF. Systemic inflammation reaction syndrome (SIRS) is a marker of ongoing inflammation. Therefore, we aim to evaluate the relationship of sterile SIRS with hepatitis B virus (HBV)-related ACLF (HBV-ACLF).

Methods: HBV-ACLF patients with sterile SIRS who were hospitalized between December 2016 and December 2018 were retrospectively analyzed. All patients were followed up until 90 days. Risk factors associated with 90-day mortality and sterile SIRS development were assessed.

Results: Among 151 HBV-ACLF patients without infection, 37 patients (24.5%) presented with or developed sterile SIRS. During the 90-day follow-up, 23 of the 37 patients with sterile SIRS died (62.2%), compared to 40 patients without sterile SIRS (35.1%, P  = 0.004). Univariate analysis showed that age, total bilirubin (TBIL), international normalized ratio, ammonia, presence of sterile SIRS, model for end-stage liver disease score, presence of complications, and organ failures were associated with 90-day mortality. In multivariate analyses, the presence of sterile SIRS was an independent risk factor for 90-day mortality. Among SIRS components, heart rate (HR) was the most frequently met criterion (56 patients, 37.09%). Patients who met the HR or temperature criterion had lower 90-day survival rate than those who did not (46.4 vs 65.3%, P  = 0.020; 16.7 vs 60.0%, P  = 0.020).

Conclusion: The presence of sterile SIRS in HBV-ACLF patients was closely associated with prognosis.

Background: Women veterans are the fastest growing group of new Veterans Health Administration (VA) users with more than half a million using VA care in 2018. VA has implemented policies and programs to meet the reproductive and sexual health (RSH) needs of women veterans who increasingly rely on VA to provide comprehensive care.

Objective: We aimed to describe changes in the prevalence of RSH diagnosis categories among women veterans using VA care in fiscal year (FY) 2010 and FY2018.

Purpose: Endoscopic lithotripsy has become widely adopted worldwide and is considered the gold standard for managing upper urinary tract lithiasis. One of its possible complications is post-operative infection. We aimed to review available literature about the role of preoperative antibiotic prophylaxis and its indications.

Transcriptional pathways of terminal differentiation in high- and low-density blood granulocytes in sepsis.

J Inflamm (Lond)

October 2024

Department of Anesthesiology, Surgical Intensive Care Medicine and Pain Medicine, Mannheim Institute of Innate Immunoscience (MI3), Medical Faculty Mannheim, Heidelberg University, Mannheim, Germany.

Article Synopsis
  • Trauma and infections trigger emergency granulopoiesis, which leads to an increase of immature granulocytes in the blood during severe conditions like sepsis, but their effectiveness as biomarkers is limited due to mixed disease severity responses.
  • * A study was conducted comparing sepsis patients to those with SIRS, using techniques like flow cytometry and gene expression assays to analyze granulocyte populations and their developmental stages.
  • * Results showed that immature granulocyte precursor counts were higher in sepsis but didn't correlate with disease severity, and that low-density granulocytes had a significantly greater number of precursors compared to high-density granulocytes.

Optimization of a Lethal, Combat-Relevant Model of Sterile Inflammation in Mice for Drug Candidate Screening.

Mil Med

August 2024

Wound Infections Department, Bacterial Diseases Branch, Center for Infectious Diseases Research, Walter Reed Army Institute of Research, Silver Spring, MD 20910, USA.

Article Synopsis
  • The study addresses the severe health risk of systemic inflammatory response syndrome (SIRS) and multiple organ dysfunction syndrome (MODS) in wounded military Service Members, particularly highlighting its prevalence in combat scenarios.
  • Researchers optimized a mouse model of lethal SIRS/MODS to test the effectiveness of immunomodulatory drugs as potential treatments.
  • Results indicated that the type of anesthesia used during the experiment influenced mortality rates, with animals receiving ketamine/xylazine (K/X) showing significantly higher lethality compared to those using isoflurane.
