Background: Thymomas are the most common anterior mediastinal malignancies, typically requiring surgical resection. While median sternotomy has been the traditional approach, robotic-assisted thymectomy using the Da Vinci Xi system is reshaping minimally invasive surgery. Its 12× magnification, 3D visualization, and articulating instruments with tremor suppression allow precise dissection in anatomically constrained areas, particularly in tumors with vascular invasion, compared with the more aggressive classical approach.

Adoption of robot-assisted coronary artery bypass grafting (RA-MIDCAB) remains limited due to concerns about learning curves, outcomes, and patient-specific anatomic challenges. This study evaluates our initial single-center experience with RA-MIDCAB. Between December 2022 and June 2024, 52 patients underwent RA-MIDCAB.

A 76-year-old man presented with new onset atrial fibrillation. During his workup, he underwent a computed tomography angiogram of his coronary arteries that revealed multivessel disease and an incidental heterogeneous fat and complex fluid density in the posterior aortopulmonary window space. The mass was further characterized with the use of magnetic resonance imaging and a dedicated computed tomography scan of the chest showing a complex fat-containing tumour most consistent with either an atypical lipomatous tumour or a well-differentiated liposarcoma.

Recent advancements in lung cancer treatment, including molecular targeted therapies and immune checkpoint inhibitors, have significantly improved patient outcomes and extended survival. However, the efficacy and safety of salvage surgery for lesions resistant to targeted therapy like osimertinib remain poorly understood. This report highlights a case of salvage surgery performed on a patient with epidermal growth factor receptor ()-mutant lung cancer resistant to osimertinib.
