Article Abstract

3DQRSarea is a strong marker for cardiac resynchronization therapy and can be obtained by taking the (i) summation or the (ii) difference of the areas subtended by positive and negative deflections in X, Y, Z vectorcardiographic electrocardiogram (ECG) leads. We correlated both methods with the instantaneous-absolute-3D-voltage-time-integral (VTI). 3DQRSarea consistently underestimated the VTI , but the summation method was a closer and more reliable approximation. The dissimilarity was less apparent in left bundle branch block (r summation .996 vs. difference .972) and biventricular paced ECGs (r .996 vs. .957) but was more apparent in normal ECGs (r .988 vs. .653).

Aims: There is a lack of data from randomized clinical trials comparing treatment outcomes between conduction system pacing (CSP) modalities and biventricular pacing (BVP) in symptomatic patients with refractory atrial fibrillation (AF) scheduled for atrioventricular node ablation (AVNA). The CONDUCT-AF investigates whether CSP is non-inferior to BVP in improving left ventricular ejection fraction (LVEF) and clinical outcomes in heart failure (HF) patients with symptomatic AF undergoing AVNA.

Methods: This study is an investigator-initiated, prospective, randomized, multicentre clinical trial conducted across 10 European centres, enrolling 82 patients with symptomatic AF, HF with reduced LVEF, and narrow QRS.

Context: Cardiometabolic complications are increasingly recognized in congenital adrenal hyperplasia (CAH) due to 21β-hydroxylase deficiency, but adult data remain limited.

Objective: To evaluate cardiovascular and metabolic alterations in adult patients with classic CAH under glucocorticoid treatment, compared to matched controls.

Methods: A cross-sectional study was conducted on adults with classic CAH and sex- and BMI-matched controls.

Background: Deep learning has significantly improved medical diagnostics, particularly in electrocardiogram (ECG) analysis, yet accurate classification of arrhythmias remains challenging.

Objective: We propose Electrocardiogram Graph Convolutional Network (ECG-GraphNet), a graph convolutional network designed to classify arrhythmias into 3 types: normal (N), supraventricular ectopic (S), and ventricular ectopic (V) beats.

Methods: ECG-GraphNet utilizes a novel graph representation of ECG data in which the P wave, QRS complex, and T wave are modeled as individual nodes.

Background: Fragmented QRS complex (f-QRS) on a 12-lead electrocardiogram (ECG) has been associated with myocardial scars. However, its diagnostic accuracy for detecting myocardial scars assessed by cardiac magnetic resonance (CMR) imaging remains uncertain.

Objective: To evaluate the diagnostic performance of f-QRS for detecting myocardial scars assessed by 3.

Background: During the electrode screwing process in left bundle branch pacing (LBBP), the significance of the S wave in lead V6 remains elusive. Our study analyzes the change of the S wave in lead V6 under different patterns of capture and explores its mechanisms.

Methods: This study included 243 cases with criterion of selective LBBP (SLBBP), we performed continuous pacing technique and classified the electrophysiological characteristics observed during the screwing process into four patterns: left ventricular septal pacing (LVSP), non-selective LBBP (NSLBBP) in low output and in the lower output, selective LBBP.
