Atherosclerosis is a life-threatening disease associated with morbidity and mortality in patients with type 2 diabetes (T2D). This study aimed to characterize a salivary signature of atherosclerosis based on evaluation of carotid intima-media thickness (IMT) to develop a non-invasive predictive tool for diagnosis and disease follow-up. Metabolites in saliva and plasma samples collected at admission and after treatment from 25 T2D patients hospitalized for 2 weeks to undergo medical treatment for diabetes were comprehensively profiled using metabolomic profiling with gas chromatography-mass spectrometry. Orthogonal partial least squares analysis, used to explore the relationships of IMT with clinical markers and plasma and salivary metabolites, showed that the top predictors for IMT included salivary allantoin and 1,5-anhydroglucitol (1,5-AG) at both the baseline examination at admission and after treatment. Furthermore, though treatment induced alterations in salivary levels of allantoin and 1,5-AG, it did not modify the association between IMT and these metabolites ( > 0.05), and models with these metabolites combined yielded satisfactory diagnostic accuracy for the high IMT group even after treatment (area under curve = 0.819). Collectively, this salivary metabolite combination may be useful for non-invasive identification of T2D patients with a higher atherosclerotic burden in clinical settings.

Introduction: Glucagon-like peptide-1 receptor agonists (GLP-1 RAs) are increasingly prescribed for Type 2 diabetes and obesity due to their cardiometabolic benefits. However, their effects on fracture healing remain controversial. This study investigates perioperative GLP-1 RA use and outcomes following surgical treatment of lower extremity (LE) fractures.

Background: Large cell neuroendocrine carcinoma (LCNEC) represents a rare and unique type of lung tumor with an unfavorable prognosis. It is essential to summarize the treatment modalities and prognosis for inoperable stage III and IV LCNEC, explore the role of frontline immunotherapy, and examine the stratification role of the Lung Immune Prognostic Index (LIPI) and its relationship with the tumor microenvironment (TME).
Methods: This study retrospectively analyzed 160 patients with inoperable lung LCNEC (L-LCNEC) admitted to three hospitals from December 2012 to November 2023.

Objective: Cinnamon-induced contact stomatitis (CCS) is a hypersensitivity reaction associated with cinnamon-containing products (CCPs). We performed a retrospective study of 74 cases of CCS induced by chewing gums or candies and reviewed the literature.
Materials And Methods: This is a retrospective study of CCS cases, diagnosed during 2010-2025, characterised by temporal association of lesions with the consumption of cinnamon-containing chewing gums or candies; lesion's resolution after discontinuation of those CCPs; and absence of recurrence as long as those CCPs were not re-used.

BACKGROUND Periprosthetic tibial fractures following total knee arthroplasty (TKA) are increasingly encountered in very elderly patients, where multiple comorbidities and osteoporosis compromise early mobilization and elevate the risk of complications. Maintaining pre-injury activities of daily living (ADL) while ensuring safe surgical management is challenging. We present a case of a 95-year-old woman with a periprosthetic tibial shaft fracture managed with open reduction, additional plate fixation, and Ilizarov external fixation, enabling immediate postoperative weight-bearing.

Objectives: Mobile medical units (MMUs) provide health care services in the community to reach populations with geographic, financial, and other barriers to care. The US Department of Veterans Affairs (VA) Homeless Patient Aligned Care Team (HPACT) program deployed MMUs to 25 sites in fiscal year 2024 to increase access for veterans experiencing homelessness. We examined early implementation of MMUs in HPACT sites by describing implementation and operational issues, services provided, and characteristics of veterans who used MMUs.
