Primary cutaneous apocrine carcinoma (PCAC) is an exceptionally rare cutaneous malignancy originating from apocrine glands, occurring most commonly in the axilla and anogenital regions. It typically follows a slow-growing clinical course, although aggressive behavior has been documented in select cases. While local recurrence and regional metastasis are not uncommon in PCAC, instances of distant metastases are rare, with only a handful of cases reported, including involvement of the liver, bone, and lung.

Background: Extramammary Paget's disease (EMPD) is a rare skin cancer that arises in areas rich in apocrine glands, such as the genital, perineal, and perianal regions. The gold standard for diagnosis is the histopathological study of a biopsy, and the standard treatment is wide surgical excision. Once the histopathological diagnosis of EMPD has been established, high frequency ultrasound (HFUS) has shown potential as a complementary non-invasive method useful for the initial assessment and EMPD's management.

Extramammary Paget's disease is a rare intraepithelial adenocarcinoma that mainly occurs in areas rich in apocrine sweat glands, such as the vulva, perianal region, and, less commonly, in the axillary skin. Owing to the varied manifestations of extramammary Paget's disease, it is often misdiagnosed. Particularly, pigmented extramammary Paget's disease (PEMPD) can be mistaken for superficial spreading melanoma, Bowen's disease, pigmented superficial basal cell carcinoma, or seborrheic keratosis.

Perianal paget's disease is an uncommon malignant diagnosis for which surgical treatment is primarily opted for. This study presents two case reports of perianal paget's disease treated with surgery in a tertiary care oncologic center in India. Case 1: A 72 year old man presented with an itchy white patch in the perianal region, with a biopsy done in 2015 revealing extra-mammary paget's disease.
