Article Abstract

Background: In high-income, temperate countries, IgE to allergen extracts is a risk factor for, and mediator of, allergy-related diseases (ARDs). In the tropics, positive IgE tests are also prevalent, but rarely associated with ARD. Instead, IgE responses to ubiquitous cross-reactive carbohydrate determinants (CCDs) on plant, insect and parasite glycoproteins, rather than to established major allergens, are dominant. Because anti-CCD IgE has limited clinical relevance, it may impact ARD phenotyping and assessment of contribution of atopy to ARD.

Methods: Using an allergen extract-based test, a glycan and an allergen (glyco)protein microarray, we mapped IgE fine specificity among Ugandan rural Schistosoma mansoni (Sm)-endemic communities, proximate urban communities, and importantly in asthmatic and nonasthmatic schoolchildren.

Results: Overall, IgE sensitization to extracts was highly prevalent (43%-73%) but allergen arrays indicated that this was not attributable to established major allergenic components of the extracts (0%-36%); instead, over 40% of all participants recognized CCD-bearing components. Using glycan arrays, we dissected IgE responses to specific glycan moieties and found that reactivity to classical CCD epitopes (core β-1,2-xylose, α-1,3-fucose) was positively associated with sensitization to extracts, rural environment and Sm infection, but not with skin reactivity to extracts or sensitization to their major allergenic components. Interestingly, we discovered that reactivity to only a subset of core α-1,3-fucose-carrying N-glycans was inversely associated with asthma.

Conclusions: CCD reactivity is not just an epiphenomenon of parasite exposure hampering specificity of allergy diagnostics; mechanistic studies should investigate whether specific CCD moieties identified here are implicated in the protective effect of certain environmental exposures against asthma.

Background: The CRP-albumin-lymphocyte (CALLY) index has potential clinical value as a novel marker integrating inflammatory, nutritional and immune status in the development of colorectal polyps. This study examined whether gender factors influence the association between CALLY and colorectal polyps; in addition to elucidating whether metabolic pathways mediate this relationship.

Methods: This is a cross-sectional study including 5409 adult health screening participants who completed colonoscopy.

Maternal systemic iron homeostasis in diabetes-in-pregnancy: A cross-sectional analytical study from a tertiary health care centre of South India.

Saudi Med J

September 2025

From the Department of Biochemistry (Thalapalliyil, Bobby), from the Department of Obstetrics and Gynecology (Dorairajan), and from the Department of Pathology (Ch. Toi), JIPMER, Puducherry, India.

Objectives: To explore whether uniform supplementation causes iron overload among a cohort of South Indian non-anemic pregnant women with diabetes-in-pregnancy (DIP).

Methods: The study took place between May 2022 and May 2024 and consisted of 120 participants from 2 groups: healthy pregnant women (HP) and pregnant women with DIP. Levels of Hb and the serum indices of iron homeostasis-iron, unsaturated iron-binding capacity, total iron-binding capacity, transferrin saturation, ferritin, hepcidin, soluble transferrin receptor (sTfR), and sTfR index, were estimated.

Purpose: The dynamic interplay between glucose metabolism and systemic inflammation is increasingly recognized as a pivotal factor influencing outcomes in older adults with community-acquired pneumonia (CAP), yet its temporal patterns and the modifying role of nutritional status remain insufficiently understood.

Patients And Methods: In this retrospective cohort study, 507 older adults (≥65 years) hospitalized with CAP were included. Serial measurements of blood glucose (GLU), C-reactive protein (CRP), and neutrophil-to-lymphocyte ratio (NLR) were obtained at admission, 24 hours, and 72 hours.

Background And Objectives: The objective of present study was to assess the relationship between the Dietary Inflammatory Index (DII) and the Glycemic Index (GI), serum TNF-α (tumor necrosis factor alpha), IL-6 (interleukin 6), serum asprosin, and omentin adipokines in prediabetic adult women.

Methods: The study included a total of 60 women: 30 women with prediabetes, aged 19-50 years, with a body mass index (BMI) ranging from 25 to 35 kg/m, and 30 healthy women with similar age and BMI as the control group. Dietary data for calculating DII and GI were obtained from food frequency questionnaires and food consumption records, respectively.

Histamine as a mediator of cross-talk between human lung mast cells and macrophages.

Histamine, a multifaceted inflammatory mediator released from mast cells and basophils, has long been recognized for its critical role in orchestrating various aspects of allergic responses. Historically, the role of macrophages in allergic disorders has been underestimated compared to other immune cells, such as mast cells, eosinophils, and basophils. These cells are the predominant immune cells in the human lung, both in healthy individuals and patients with asthma.
