Article Abstract

The original Gillquist maneuver is done by passing the arthroscope through a portal in the patella tendon between the medial femoral condyle and posterior cruciate ligament to enter the posterior compartment. This is done blind and has been documented to result in broken cameras and damaged equipment. It is also necessary to do a notchplasty to aid the advancement of the camera in patients. In our paper, we have made modifications to allow the Gillquist maneuver to be done safely under direct visualization, with just the aid of a simple switching stick. Our technique starts with the arthroscope in the anteromedial portal. We insert a long, cannulated switching stick through the anterolateral portal and pass it between the medial femoral condyle and the posterior cruciate ligament. The switching stick, being tapered and narrow, is able to traverse the transcondylar notch with minimal trauma. Once the switching stick enters the posterior compartment, the camera and trocar are removed and the trocar sleeve is guided over the switching stick past the intercondylar notch gently. The switching stick is then replaced by the arthroscope, which is advanced through the trocar sleeve and into the posterior compartment.

Background: The potential for heated tobacco products (HTPs) to reduce smoking-related harm depends, in part, on how adults who smoke cigarettes use HTPs in their everyday lives, and the extent to which HTPs come to be used as a replacement for all or most of the cigarettes that a person smokes. This study assessed changes in cigarette smoking behaviour among adults who smoke when using HTPs in near-to-real world settings for six weeks.

Methods: Participants were 332 adults aged 19 years and older who smoked between 5 and 30 cigarettes per day, on average, living in Prague or Brno, Czechia, who reported no intention to quit smoking within the next three months but reported a positive likelihood of using the PULZE + iD Heated Tobacco System ('the Study Product') on a regular basis following a brief trial use period.

Facultative parthenogenesis enables females to switch from asexual (parthenogenetic) to sexual reproduction after mating, but the process of fertilisation is poorly understood in such animals. In particular, it is not known whether switching reproductive modes requires changes in the eggs themselves, delaying the transition from laying unfertilised to fertilised eggs. Symptomatic small rotator cuff tears are a common issue for orthopedic surgeons. While arthroscopic repair is the standard treatment after conservative methods fail, no consensus exists on the ideal technique. This study presents a rotator cuff tissue-preserving repair method using a switching stick and spinal needle, which is reproducible, easy to learn, and ensures equal tendon tissue tension post-repair.
