A conjoint twin is a rare congenital anomaly. The separation of conjoint twins presents a unique challenge to anesthesiologists. We report the anesthetic management of successful separation of thoraco-omphalopagus conjoint twins, along with challenges and limitations that we faced in perioperative period. We highlight the importance of a multidisciplinary approach, proper preoperative planning, and perioperative vigilance regarding monitoring and complication.

Background: The birth of conjoined twins is an extremely rare neonatal condition, occurring in approximately 1 in 100,000 live births. Ischiopagus, a form of conjoined twinning where the twins are joined at the pelvis, accounts for 6-11% of all conjoined twins. Ischiopagus conjoined twinning is a congenital anomaly.

Twin pregnancy is considered high-risk pregnancy because of its various effects on maternal and fetal physiology. Twin pregnancy can be dizygotic or monozygotic, the latter being less common. Depending upon the period of separation of an embryo, monozygotic twins can share amniotic cavity and placenta with the monochorionic monoamniotic form being the least common type.

Background: The increased application of immune checkpoint inhibitors (ICIs) targeting PD-1/PD-L1 in lung cancer treatment generates clinical need to reliably predict individual patients' treatment outcomes.
Methods: To bridge the prediction gap, we examine four different mathematical models in the form of ordinary differential equations, including a novel delayed response model. We rigorously evaluate their individual and combined predictive capabilities with regard to the patients' progressive disease (PD) status through equal weighting of model-derived outcome probabilities.

Conjoined twins are described as having been physically fused during pregnancy and delivery. They were first mentioned a long time ago when there was not much known about this. They share some organs that are vital for survival, like the heart; these twins are almost impossible to save, but there are some cases wherein there is evidence of their survival.

Being unique is good, but not so for conjoined twins. Cephalothoracopagus is a rare type of conjoined twins, which is a rare monozygotic twinning resulting in imperfect fusion of the head, chest and upper abdomen. These twins have separated limbs and pelvis.
