Article Abstract

Despite advances in antibiotic and surgical management and supportive care for necrotizing soft tissue infections, morbidity and mortality remain substantial. Although there are clinical practice guidelines in place, there still remains much variability in choice and duration of antibiotic therapy, time to initial surgical debridement, and use of adjuvant medical therapies. This article offers an overview of necrotizing soft tissue infections with a focus on current diagnostic and treatment modalities.

Skin and soft tissue infections (SSTIs) represent a diverse spectrum of conditions, including erysipelas, cellulitis, cutaneous abscesses, necrotizing fasciitis, and myonecrosis. Erysipelas and cellulitis are the most common community-acquired SSTIs. Erysipelas is typically caused by pyogenic streptococci, while cellulitis often has a staphylococcal etiology.

Background: Soft-tissue filler injection-related vascular complications (IRVC) can lead to tissue necrosis, vision loss, and stroke. Hyaluronidase-based injections are the mainstay of treatment, though other options include heat, massage, nitroglycerin, and anticoagulants. Given the narrow therapeutic window and limited early warning signs, we propose training residents to implement a standardized soft-tissue filler vascular complication (SFVC) management protocol.

Fournier's gangrene is a severe necrotizing soft tissue infection requiring urgent debridement and broad-spectrum antibiotics with large defects posing cosmetic and functional challenges. This review presents considerations and possibilities for reconstruction of the scrotum after heavy debridement, mainly focusing on split-thickness skin grafts and local flap techniques. While flaps provide superior cosmetic and protective outcomes, grafts are simpler and more cost-effective.
