Complete atrioventricular (AV) block is defined as a dissociation of atrial and ventricular activities. Complete AV block that occurs during the perioperative period is difficult to reverse and usually requires implantation of a pacemaker. Propofol does not affect a normal AV conduction system but may act as a trigger for AV block. It can also potentiate vagal stimulation factors and reduce sympathetic activity. We report a case of complete AV block that may have been related to administration of propofol.

Objective: To evaluate the efficacy of baricitinib in combination therapy for managing refractory, rapidly progressive systemic sclerosis (SSc) with severe cardiac conduction defects and interstitial lung disease (ILD).
Methods: A 48-year-old male patient with SSc complicated by significant cardiac enlargement, third-degree atrioventricular block, heart failure, progressive ILD, and partial intestinal obstruction was included in the study. Prior treatments with mycophenolate mofetil (MMF), tacrolimus, and cyclophosphamide (CTX) had shown limited efficacy.

Objective: To evaluate the impact of CT planning on surgical myectomy outcomes in patients with hypertrophic cardiomyopathy (HCM) and left ventricular outflow tract (LVOT) and/or mid-cavity obstruction, by comparing these outcomes with those of conventional surgical myectomy.
Methods: This prospective cohort study included patients who underwent surgical septal myectomy for HCM with LVOT and/or mid-cavity obstruction between January 2019 and May 2024 at a single tertiary center. In the CT-planned myectomy group, an expert radiologist simulated the target myectomy site through a series of post-processing methods to plan the surgical approach, provide a surgeon's view that closely resembles the actual perspective in the operating room, and present the target myectomy volume.

Background: The Wenckebach phenomenon of atrioventricular conduction is deemed a physiological response of the atrioventricular node to a high atrial rate. However, the reverse Wenckebach phenomenon is under-recognized.
Case Summary: We report an instance of reverse Wenckebach phenomenon developing into complete heart block in a 33-year-old man who experienced syncope after surgical repair of prolapsed bicuspid aortic valves.

Background: Becker muscular dystrophy (BMD) is frequently associated with cardiac involvement. The underlying pathoanatomical substrate includes replacement of cardiomyocytes by fibrous tissue, leading to extensive myocardial fibrosis of the posterolateral wall of the left ventricular (LV) epicardium. Cardiac arrhythmias, including ventricular tachycardia (VT), are common in this condition, particularly when LV ejection fraction (LVEF) declines.

Objective: Although outcomes of repair of complete atrioventricular septal defect (cAVSD) have improved in recent years, the burden of reoperation on the left atrioventricular valve (LAVV) remains high. We investigated predictors of LAVV reoperation on the intra-operative echocardiogram that may help guide surgical decision making.
Methods: All patients who underwent repair of cAVSD at the Royal Children's Hospital from 2010 to 2020 were included in the study.
