Internal duplication can enhance the function of a gene or provide raw material for the emergence of a new function in a gene. Therefore, it is interesting to see whether the frequency of internal duplication has increased during metazoan evolution. The growing number of sequenced eukaryotic genomes provides an excellent opportunity to study the change in the pattern of internal duplication in the course of metazoan evolution. We studied repeated segments in proteins in the proteomes of 11 eukaryotes. We found that the frequency of internal duplication in Caenorhabditis elegans and Drosophila melanogaster (two protostomes) is higher than that in fungi but lower than that in chordates. Moreover, the frequencies of internal duplication for the chordates studied are largely similar. We classified orthologous proteins of chordates into three antiquity groups and found that more recently derived proteins in the metazoan lineage have higher repetitiveness than older ones. Our analysis suggests that lineage-specific internal duplication in protein evolution increases with organismal complexity before the emergence of chordates but not so afterward. Proteins with repeated regions might have been preferred before the protostome-chordate split. This finding supports the suggestion that exon-shuffling occurred more frequently after the first multicellular organism appeared and might have contributed to the metazoan radiation.

Antimicrobial resistance is an urgent public health threat, and despite significant consumption of antimicrobials in pregnancy, there remain opportunities for improvement of their use in the obstetric population. Improvement in antimicrobial utilization can be streamlined by assessing baseline characteristics, utilization of diagnostic testing, awareness of peripartum protocols, and recognition of penicillin allergies. In a single healthcare system including 8 obstetric hospitals, an administrative review identified 199 different regimens used among 8,528 patients based on American College of Obstetrician and Gynecologists (ACOG) guidelines.

Background: In the United States, cancer is more prevalent in racial and ethnic minority groups and in rural-dwelling and low-income people. Compared with White people of non-Hispanic descent, Black and African American people have higher cancer mortality and Hispanic people are more likely to be diagnosed with infection-related cancers. In addition, people who live in persistent poverty areas are more vulnerable to cancer mortality.

BACKGROUND Duplicated internal auditory canal (dIAC) is a rare congenital temporal bone anomaly associated with ipsilateral sensorineural hearing loss (SNHL). The Bonebridge bone conduction implant has a magnet, an internal transducer, and an external audio processor. This report is of a 14-year-old girl with unilateral SNHL and vestibulocochlear nerve (VIII cranial nerve) aplasia due to dIAC who was treated with a Bonebridge bone conduction implant.

Objectives: Traumatic brain injury (TBI) is a significant contributor to long-term disability and imposes substantial costs, which may include excess, duplicative, or unnecessary imaging, hospitalizations or transfers, and superfluous consultations. We examined the use of the Brain Injury Guidelines (BIG) in 144 consecutive patients presenting with TBI.
Methods: Patients with TBI evaluated consecutively between April 2024 and September 2024 were prospectively classified into BIG 1, 2, and 3 categories and retrospectively assessed.
