Introduction: Chronic volar wrist swelling is a diagnostic challenge, particularly in tuberculosis-endemic regions, where both infectious and inflammatory causes must be considered. Rheumatoid tenosynovitis, an extra-articular manifestation of rheumatoid arthritis (RA), can mimic infectious tenosynovitis, especially in seronegative patients.

Case Report: We report a case of a 44-year-old male presenting with progressive pain and swelling over the volar aspect of the right wrist for 2 months.

Context: Neurogenic bladder is one of the major complications of spinal cord injury (SCI). Due to inadequate, ill-timed intervention, it can be lethal and lead to chronic case of renal calculus, renal failure or recurrent urinary tract infections. Intermittent theta burst stimulation (iTBS) is a patterned, non-invasive transcranial magnetic stimulation procedure that can improve the functional status of patients with SCI by manipulating cortico-spinal plasticity.

Individuals with hereditary angioedema (HAE), a rare disease most frequently associated with deficiency (HAE-C1INH-Type1) or dysfunction (HAE-C1INH-Type2) of C1 inhibitor (C1INH), continue to experience frequent misdiagnoses and long delays in diagnosis, preventing appropriate management strategies and placing the patients at continued risk of inappropriate management of painful, debilitating, and potentially fatal swelling attacks. Physician education to increase HAE awareness is important to initiate diagnostic testing for patients who may be at risk of HAE. Standard tests for diagnosing HAE-C1INH-Type1 and HAE-C1INH-Type2 include measurements of antigenic C4 level, antigenic C1INH level, and C1INH function; in contrast, known subtypes of HAE due to normal C1INH can only be confirmed through genetic testing.

Introduction/aims Of Study: This original work done in patients with Benign Prostatic Obstruction aims to find out the impact of duration of medication therapy on the outcomes of TURP. Medical management of BPO is recommended as the first line of treatment and surgery is usually reserved for those who have failure of medical management and/or complications of BPO (refractory retention, vesical calculus, recurrent UTI or hematuria). However, there is some pointing evidence that delay in the surgery may affect the overall outcomes of transurethral resection of prostate (TURP) [1, 2] Presently, there is a lacuna on the impact of delay in surgery owing to prolonged duration of preoperative medical therapy on the outcomes of surgery.

The effects of adding circumferential groove geometries on the collapse capacity of carbon composite cylindrical tubes were investigated experimentally and numerically. Tubular specimens, both with and without grooves, were imploded hydrostatically in a water-filled pressure vessel facility. High-speed imaging captured the collapse behavior, while dynamic pressure transducers recorded the transient collapse pressure history of the implosion event.

Purpose: Standard of care for patients with sickle cell disease (SCD) includes red blood cell transfusions (RBCTs). Data on clinical and economic outcomes of patients with SCD receiving frequent RBCTs are limited.

Materials And Methods: This longitudinal, retrospective, claims-based analysis used the Merative™ MarketScan Commercial, Medicare, and Multi-State Medicaid databases.

Background: Symptoms of hereditary angioedema (HAE) typically first present during childhood, but the frequency/severity of attacks often increases at puberty. Real-world data on long-term HAE prophylaxis in adolescents are limited. We report pooled data from adolescent patients enrolled in two Phase 4 studies (EMPOWER, ENABLE) evaluating the effectiveness/safety of lanadelumab (monoclonal antibody directed against plasma kallikrein) for the prevention of HAE attacks.

Bone marrow aspiration is an essential procedure for diagnosing hematological conditions and therapeutic applications, such as bone marrow aspirate concentrate (BMAC) therapy for non-union of bones. This report presents a case of a 35-year-old male with femoral non-union treated with BMAC, requiring the collection of a large bone marrow aspirate volume. A novel, low-cost modification to the standard bone marrow aspiration apparatus was developed using components from an intravenous (IV) drip set, including a rubber bulb connector and a luer lock connector.

Background: The role of repeat transurethral resection of bladder tumor (TURBT) for the management of nonmuscle invasive bladder carcinoma is debated, especially when initial resections include detrusor muscle. This study compares immediate second resection (additional deep biopsies in the same session) with standard restage TURBT performed 2-6 weeks post-initial TURBT to determine adequacy in detrusor muscle sampling and compare the disease rate at restage TURBT in both groups.

Material And Methods: A randomized trial was conducted at a tertiary care hospital, including patients aged ≥18 years undergoing TURBT with complete primary tumor resection.

Chimeric antigen receptor T-cell (CAR-T) therapy has revolutionized the treatment of various hematological malignancies. Recently, CAR-T has been used in refractory auto-immune diseases with initial encouraging results. In this systematic review, we examined the safety and efficacy of CAR-T in patients with refractory auto-immune diseases.

Immunoglobulin G4 (IgG4)-related disease is a rare immune-mediated fibro-inflammatory disorder with many clinical manifestations. Most commonly affected sites include the pancreas, salivary glands, kidneys, lachrymal glands, biliary tree, aorta and retroperitoneum; testicular involvement is a rare entity. This report describes a man in his 20s who presented with a persistent painless mass in his left testis.

ObjectiveNeonatal seizures present significant treatment challenges, often requiring adjunctive antiseizure medications. This study aimed to evaluate the efficacy of lacosamide as an adjunctive antiseizure medication in managing refractory neonatal seizures.MethodsThe study included neonates up to 44 weeks of corrected gestational age who received lacosamide treatment at a level 4 neonatal intensive care unit between January 2015 and December 2023.

Introduction: Enchondromas are most commonly found in the bones of the hand and feet. Although the location in the feet is not uncommon the enchondroma of the calcaneus is a rare finding.

Case Report: In this case report, we present a 45-year-old male patient who was diagnosed with enchondroma of the calcaneum.

Background: The ACR in 2021 and the EULAR in 2022 published recommendations for management of ANCA-associated vasculitis. Given the differences in the demographic, clinical profiles, and the socio-economic realities between various countries, there is a need for development of guidelines for the management of AAV for less economically developed regions of the world.

Methods: These guidelines were made following the GRADE methodology.

Gait impairment and neurogenic bladder are co-existing common findings in incomplete spinal cord injury (iSCI). Repetitive transcranial magnetic stimulation (rTMS), evident to be a promising strategy adjunct to physical rehabilitation to regain normal ambulation in SCI. However, there is a need to evaluate the role of Intermittent theta burst stimulation (iTBS), a type of patterned rTMS in restoring gait and neurogenic bladder in SCI patients.

Background: Dermatoscopy is a noninvasive method of examining skin lesions under high magnification, gradually replacing the need for invasive biopsies. Training is required to gain clinical competency. Gamification employs game-like elements to enhance education engagement and is an engaging means of delivering medical education.

Background: Observational studies of anesthetic neurotoxicity may be biased because children requiring anesthesia commonly have medical conditions associated with neurobehavioral problems. This study takes advantage of a natural experiment associated with appendicitis to determine whether anesthesia and surgery in childhood were specifically associated with subsequent neurobehavioral outcomes.

Methods: This study identified 134,388 healthy children with appendectomy and examined the incidence of subsequent externalizing or behavioral disorders (conduct, impulse control, oppositional defiant, attention-deficit hyperactivity disorder) or internalizing or mood or anxiety disorders (depression, anxiety, or bipolar disorder) when compared to 671,940 matched healthy controls as identified in Medicaid data between 2001 and 2018.
