Background: Timely palliative transition in patients with advanced cancer is essential for their improved quality of life and overall survival (OS). Most prognostic models have been developed focusing on weeks' survival. The current study aimed to compare the accuracies of several indicators, such as the Karnofsky Performance Scale (KPS), Clinicians' Prediction of Survival (CPS), and Edmonton Symptom Assessment System (ESAS), for predicting the survival of patients.

Article Synopsis
  • Recent guidelines suggest clinicians' prediction of survival (CPS) in advanced cancer patients is often inaccurate, leading to either overestimation or underestimation of the actual survival time.
  • A study analyzed data from 2,571 patients across multiple palliative care units in Asia to identify factors affecting CPS accuracy, discovering that older age and reduced oral intake were linked to underestimation, while dyspnea and hyperactive delirium were associated with overestimation.
  • Findings indicate that age influences survival prediction inaccuracies differently, with older patients being more likely to have their survival underestimated, but reduced oral intake does not necessarily correlate with this underestimation.

The Republic of Korea Navy (ROKN) has applied Occupational Health and Safety Management System (OHSMS), but the effectiveness of OHSMS is being questioned due to continuous industrial accidents that continue to occur. Although OHSMS, which has been generally applied in enterprises, has more potential for improper implementation in the military, there are few studies on OHSMS in the military. Therefore, this study verified the effectiveness of OHSMS in the ROKN and derived improvement factors.

Article Synopsis
  • The study evaluated the effectiveness of the Objective Prognostic Score (OPS) and the Palliative Prognostic Score (PaP) in predicting survival in terminal cancer patients across palliative care units in Japan, Korea, and Taiwan.
  • OPS does not require clinicians to predict patient survival, making it potentially more accessible for inexperienced doctors, unlike PaP which requires a clinical prediction of survival.
  • Both scoring methods demonstrated solid accuracy; however, OPS showed particular promise due to its ease of use, which could encourage wider adoption among less experienced healthcare providers.

Background: We aimed to investigate the performance of clinician prediction of survival (CPS) and the association between CPS and the prognostic confidence of clinicians in ambulatory medical oncology outpatients.

Methods: Eight medical oncologists estimated the expected survival of their patients in a prospective cohort study. They were asked to provide a temporal type of CPS in weeks, together with their level of confidence for each prediction (0-100%).

Article Synopsis
  • The study aimed to understand the symptoms and signs present in the last days of life for patients with advanced cancer, noting variations based on cancer type.
  • Researchers analyzed data from 2,131 patients in Japan, Korea, and Taiwan, finding that while symptoms remained stable in the week leading up to death, specific symptoms like edema and respiratory issues varied by cancer type.
  • The findings help clinicians better recognize and manage end-of-life symptoms, improving care for patients facing advanced cancer.

Article Synopsis
  • The study aimed to compare clinicians' predictions of survival with Palliative Prognostic Scores (PaP) in patients with advanced cancer across Japan, Korea, and Taiwan.
  • The researchers analyzed data from 2,072 patients to assess the accuracy of different predictive models for 30-day survival using a statistical method called AUROC.
  • Results showed that the PaP total scores and the PaP-CPS had similar predictive accuracy, suggesting that experienced clinicians might rely on the CPS alone to estimate short-term survival for advanced cancer patients.

Symptoms are not typically part of established various prognostic factors and scoring systems but are among the most frequently assessed issues in patient care. To evaluate that, changes in symptoms can provide additional useful prognostic information. A secondary analysis of an international cohort study in Japan, Korea, and Taiwan.

Background: The simplified Palliative Prognostic Index (sPPI) substitutes a single item from the Communication Capacity Scale (CCS) for the delirium item of the original PPI. This study aimed to examine the validity of the sPPI for patients with advanced cancer in a home-based hospice care setting.

Methods: This study included 75 patients with advanced cancer who received home-based hospice care.

Objective: Several studies supported the usefulness of "the surprise question" in terms of 1-year mortality of patients. "The surprise question" requires a "Yes" or "No" answer to the question "Would I be surprised if this patient died in [specific time frame]." However, the 1-year time frame is often too long for advanced cancer patients seen by palliative care personnel.

Context: Spiritual well-being (SWB) is significant for patients with life-limiting illnesses. Thus, shortened versions of questions would be helpful in approaching SWB.

Objectives: Our goal was to develop a one-item screening question to assess the SWB of advanced cancer inpatients.

Barriers to Early Palliative Care.

Hanguk Hosupisu Wanhwa Uiryo Hakhoe Chi

December 2020

This article aims to discuss the barriers hindering cancer patients from receiving early palliative care, which has been demonstrated to be more effective in improving quality of life and controlling symptoms. Specifically, there are barriers in four aspects of delivering early palliative care. First, the difficulty of starting discussions about early palliative care and the lack of adequate appointment time can impede communication between oncologists and patients and their family members.

Context: Previous studies suggest that clinicians' prediction of survival (CPS) may have reduced the accuracy of objective indicators for prognostication in palliative care.

Objectives: We aimed to examine the accuracy of CPS alone, compared to the original Palliative Prognostic Score (PaP), and five clinical/laboratory variables of the PaP in patients with far advanced cancer.

Methods: We compared the discriminative accuracy of three prediction models (the PaP-CPS [the score of the categorical CPS of PaP], PaP without CPS [sum of the scores of only the objective variables of PaP], and PaP total score) across 3 settings: inpatient palliative care consultation team, palliative care unit, and home palliative care.
