Purpose: Despite its importance, little is known about the patterns and predictors of Survivorship Clinic attendance in head and neck cancer (HNC). We sought to determine the cumulative incidence of Survivorship Clinic attendance stratified by demographic, clinical, and socioeconomic factors, and to identify factors independently associated with attendance.

Methods: Our analysis population consisted of 2,252 patients diagnosed with primary HNC and seen at our institution's HNC Survivorship Clinic after completing treatment from 2016-2021.

Purpose Of Review: Two-team approaches to head and neck cancer ablation and free flap reconstruction now occur concurrently in most centers. While the functional outcomes after head and neck ablation and reconstruction are typically examined through the lens of the reconstructive surgeon, optimizing the patient's functional outcome requires careful considerations of the entire surgical team. This review summarizes important intraoperative considerations for decision making by the surgical team, with a particular emphasis on ablative considerations, to optimize reconstructive outcomes after tumor extirpation.

Purpose Of Review: We briefly review the history of surgical innovation in free flap reconstruction of the head and neck. With a specific focus on maximizing patient QoL by prioritizing a 'precision reconstruction', we then posit novel avenues for surgical refinement to usher in the next generation of head and neck free flap reconstruction.

Recent Findings: Patients undergoing free flap reconstruction of the head and neck increasingly co-prioritize preservation of quality-of-life (QoL) alongside the desire to survive their diagnosis.

Importance: While neoadjuvant chemotherapy for head and neck squamous cell carcinoma dates to the earliest multidisciplinary approaches, the introduction of immune checkpoint inhibitors (ICIs) has renewed enthusiasm and research into its use. The combination of cell-free DNA (cfDNA) and digital droplet PCR (ddPCR) has significantly advanced the noninvasive screening, diagnosis, and monitoring of diseases, enabling highly sensitive and absolute quantification of target nucleic acids even in the presence of high background DNA. However, widespread adoption of ddPCR is hindered by higher costs, extended processing times, and the requirement for cfDNA purification, which adds expense and variability. We leverage a clinical trial (NCT04080804) that compared neoadjuvant anti-PD-1, anti-PD-1+CTLA-4, and anti-PD-1+LAG-3 therapies in head and neck squamous cell carcinoma patients. Combination therapies promote higher pathologic response rates versus monotherapy, and major pathologic response is associated with better survival.
