Background Patients with heart failure (HF) often face the need to maintain stable symptoms after discharge. Self-care management, including symptom monitoring, is recommended; however, because of factors such as population aging, these self-care behaviors are difficult to maintain for some patients, and the re-hospitalization rate remains high in Japanese registries. Objective In this retrospective cohort study, we aimed to verify whether the use of a self-care management tool among hospitalized patients with HF in a regional city hospital contributes to the reduction of cardiovascular events within a period of six months.

Introduction Poor physical performance is associated with an increased risk of post-discharge cardiovascular events in patients with heart failure. Cardiac resynchronization therapy (CRT) is an effective treatment of heart failure (HF) with ventricular dyssynchrony, but not all patients respond to a similar extent. We investigated the efficacy and safety of exercise training (ET) in patients without response to CRT. We introduced an attempt at cancer rehabilitation at Osaka Medical College Hospital. We also reported trends in the clinical department that ordered the cancer rehabilitation, and the days needed to consult the rehabilitation department after hospitalization for 1,028 patients who needed rehabilitation from January to June 2012. The number of rehabilitation orders for cancer patients has increased in comparison with the same period during 2009, and the percentage of cancer rehabilitation orders has also increased, both in total and in each clinical department consulted.
